针药结合对脑卒中肢体痉挛大鼠大脑皮质BDNF、TrkB、GABAa受体表达的影响 被引量:9

Effects of Acupuncture Combined with Medicine on Expression of BDNF,TrkB and GABAa Receptors in Cerebral Cortex of Rats with Limb Spasticity After Stroke

摘要 目的:观察针药结合对脑卒中肢体痉挛大鼠大脑皮质脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)、酪氨酸蛋白激酶受体B(tyrosine kinase receptor b,trkB)、γ-氨基丁酸a型(gamma-aminobutyric acid type a,GABAa)受体表达水平的影响。方法:63只实验大鼠通过2次随机分为空白组(不做任何处理)、假手术组(干预同模型组只分离不结扎不插线)、模型组(采用大脑中动脉线栓结合内囊注射N-methyl-D-aspartate,NMDA受体制作脑卒中肢体痉挛大鼠模型)、电针组(电针阳陵泉、曲池)、中药组(加味芍药甘草汤水煎剂灌胃)、针药组(电针阳陵泉、曲池+加味芍药甘草汤水煎剂灌胃)和西药组(巴氯芬溶液灌胃)7组各9只。各治疗组大鼠成模后第1天开始干预连续5 d,观察各组大鼠的神经功能和肌张力评分,检测大脑皮质中BDNF、TrkB、GABAa受体基因和蛋白的表达水平。结果:与假手术组比较,模型组大鼠神经功能和肌张力评分上升明显(P<0.01),BDNF、TrkB和GABAa受体基因和蛋白表达水平均出现明显下降(P<0.01);经治疗后各组大鼠上述指标均有明显改善(P<0.05,P<0.01),其中针药组效果优于电针组和中药组(P<0.05,P<0.01)。结论:各治疗方法均可改善模型大鼠神经功能,缓解卒中后肢体痉挛,针药结合方案优于单纯电针和中药疗法,其机制可能与上调大脑皮质中BDNF、TrkB、GABAa受体基因和蛋白表达水平有关。 Objective:To observe the effects of acupuncture combined with medicine on Brain-Derived Neurotrophic Factor(BDNF),Tyrosine kinase receptor B(TrkB),Gamma-Aminobutyric acid type a(GABAa)receptors in cerebral cortex of rats with Limb Spasticity After Stroke(LSAS).Methods:63 SD rats were twice randomly selected and equally divided into 7 groups with 9 rats in each group:Blank group(without any intervention),Sham-operation group(the same intervention as the model group,only being separated without ligation and plugging),Model of LSAS group(injecting methyl-D-aspartate,NMDA receptor into internal capsule with modified Zea Longa thread embolus method),Electro-acupuncture treatment group(electro-acupuncture on Yang lingquan and Quchi)and Chinese Medicine group(modified Shaoyao Gancao Decoction for gavage),Acupuncture combined with medicine group(electro-acupuncture on Yang lingquan and Quchi+modified Shaoyao Gancao Decoction for gavage)and Western medicine group(baclofen solution for gavage).Each treatment group was intervened on the first day after the successful establishment of the model,and continued for 5 days.The scores of neural function and muscle tension of rats in each group were observed and the expression of BDNF,TrkB,GABAa receptors in cerebral cortex were detected.Results:Compared with the sham-operation group,the scores of neural function and muscle tension in the model group increased significantly(P<0.01),and the expression levels of BDNF、TrkB and GABAa receptor genes and proteins were significantly decreased(P<0.01);after treatment,the above indexes of the rats in each group were significantly improved(P<0.05,0.01).and the effect of Acupuncture combined with medicine group was better than that of electro-acupuncture group and Chinese medicine group(P<0.05,P<0.01).Conclusion:All the treatment methods can improve the neural function in model rats,and relieve limb spasictiy caused by stroke,the curative effect of acupuncture combined with medicine is better than simple use.The mechanism may be related to
