摘要
目的:探讨肺炎患儿血清维生素A(VA)、维生素D(VD)水平与疾病严重程度的关系。方法:选取2016年11月至2017年3月于重庆医科大学附属儿童医院呼吸病房住院治疗的256例3岁以下的肺炎患儿为研究对象,入院时检测患儿血清VA、VD水平,按病情严重程度分为普通肺炎组(n=200)、重症肺炎组(n=56),并收集相关临床资料。结果:256例肺炎患儿血清VA、VD水平分别为(0.84±0.37)μmol/L、(48.11±17.55)nmol/L,其中VA缺乏(VAD)、VD缺乏的检出率分别为46.5%、58.2%;重症肺炎组VA、VD水平低于普通肺炎组,VAD、VD缺乏检出率升高(P<0.05);不同VA水平的肺炎患儿VD缺乏检出率比较差异有统计学意义(P<0.05),VAD患儿VD缺乏检出率更高;VD缺乏的肺炎患儿住院时间长于VD不足组和VD正常组(P均<0.05)。结论:肺炎患儿VA、VD水平高低与肺炎发生及严重程度相关。
Objective:To probe into the correlation between serum levels of vitamin A(VA)and vitamin D(VD)and disease severity in children with pneumonia.Methods:A total of 256 children with pneumonia under 3 years old admitted into the respiratory ward of Children’s Hospital of Chongqing Medical University from Nov.2016 to Mar.2017 were extracted.Serum levels of VA and VD were measured on admission,and all patients were divided into the common pneumonia group(n=200)and the severe pneumonia group(n=56)according to the severity of the disease.The related clinical data were collected.Results:The serum levels of VA and VD in 256 children with pneumonia were respectively(0.84±0.37)μmol/L and(48.11±17.55)nmol/L,the detection rates of VA deficiency(VAD)and VD deficiency were respectively 46.5%and 58.2%.The serum levels of VA and VD in the severe pneumonia group were lower than those in the common pneumonia group,and the detection rate of VAD and VD deficiency increased(P<0.05).The difference in the detection rate of VD deficiency in children with pneumonia at different VA levels was statistically significant(P<0.05),and the detection rate of VD deficiency was higher in children with VAD.The length of stay in children with pneumonia with VD deficiency was longer than that of the VD deficiency group and the VD normal group(P<0.05).Conclusion:The serum levels of VA and VD in children with pneumonia is related to the occurrence and severity of pneumonia.
