儿童20556例食物过敏原与19453例吸入过敏原定量检测及流行特征分析 被引量:11

Analysis on quantification detection and epidemic characteristics among 20556 cases of food allergens and 19453 cases of inhalant allergens in children

摘要 目的探讨儿童特异性过敏原的检测结果分布和其规律,为儿童过敏性疾病的预防和诊疗提供科学的参考依据。方法应用酶联免疫捕获法,定量检测过敏原特异性免疫球蛋白E(IgE),对2019年1月至2020年3月就诊于天津市儿童医院患儿的20556例食物过敏原组合与19453例吸入过敏原组合的数据进行统计分析。通过大量数据,分析不同性别、年龄儿童的过敏原流行特征。结果食物过敏原组合阳性率64.05%(13167/20556),吸入过敏原组合阳性率50.47%(9818/19453),食物性过敏原阳性率前三位依次是鸡蛋(49.71%,10219/20556)、牛奶(29.43%,6050/20556)、小麦面粉(18.92%,3889/20556),吸入性过敏原阳性率前三位依次是屋尘(31.00%,6031/19453)、交链孢霉(21.62%,4206/19453)、粉尘螨(12.59%,2450/19453),食物过敏原组合中不同性别阳性率为男63.08%(7845/12436),女65.54%(5322/8120),吸入过敏原组合中不同性别阳性率为男51.92%(6145/11835),女48.21%(3673/7618),食物过敏原组合中不同年龄组阳性率为<1岁42.54%(1401/3293),1~3岁78.13%(5730/7334),4~6岁68.37%(4078/5965),>6岁49.39%(1958/3964)。吸入过敏原组合中不同年龄组阳性率为<1岁19.72%(489/2480),1~3岁49.55%(3459/6981),4~6岁58.03%(3468/5976),>6岁59.81%(2402/4016)。结论不同性别、年龄组中过敏原阳性率有差异,有必要视流行病学特征,合理规避高风险过敏原,对过敏性疾病的预防、诊断和治疗有着重要的临床意义。 Objective To investigate the detection results distribution and rule of children′s specific allergens in order to provide the scientific basis for the prevention,diagnosis and treatment of child allergenic diseases.Methods The data in 20556 children cases of food allergen combination and 19453 children cases of inhalatant allergen combination in Tianjin Municipal Children′s Hospital from January 2019 to March 2020 were statistically analyzed by using the enzyme-linked immunocapture method and allergen specific IgE detection.The allergen epidemiological characteristics were analyzed based on the children′s characteristics and factors such as different sexes and ages and by the mass data.Results The positive rate of food allergen combination was 64.05%(13167/20556),which of inhalant allergen was 50.47%(9818/19453).The top three positive rates of food allergens were egg 49.71%(10219/20556),milk 29.43%(6050/20556)and wheat flour 18.92%(3889/20556).The top three positive rates of inhalant allergens were house dust 31.00%(6031/19453),alternaria 21.62%(4206/19453)and dermatophagoides farinae 12.59%(2450/19453).The positive rates of different genders in food allergen combination were male 63.08%(7845/12436),female 65.54%(5322/8120).The positive rates of different genders in inhalant allergens were male 51.92%(6145/11835)and female(48.21%(3673/7618).The positive rates of food allergens in different age groups were 42.54%(1401/3293)in<1 year old,78.13%(5730/7334)in 1-3 years old,68.37%(4078/5965)in 4-6 years old and 49.39%(1958/3964)in>6 years old.The positive rates of inhalant allergens in different age groups were 19.72%(489/2480)in<1 year old,49.55%(3459/6981)in 1-3 years old,58.03%(3468/5976)in 4-6 years old and 59.81%(2402/4016)in>6 years old.Conclusion The positive rate of allergens has the difference between different genders and among different age groups.It is necessary to reasonably avoid the high risk allergens according to the epidemiological characteristics,which has an important significance for the pre
