摘要
目的:建立大鼠坐骨神经牵拉损伤动物模型,观察神经牵拉损伤后大鼠坐骨神经功能,电生理及病理学变化。方法:24只清洁级健康大鼠以简单随机化分为对照组(A),轻(B)、中(C)、重度(D)牵拉组4组,每组6只。选择左侧坐骨神经暴露,A组暴露神经10 min后关闭伤口,B、C和D组使用不同拉力牵拉坐骨神经干10 min后关闭伤口。分别检测各组损伤肢体坐骨神经功能指数、滚轮运动试验及热刺激试验、神经靶肌肉复合肌肉动作电位及坐骨神经牵拉节段组织病理学变化。组间比较采用t检验。结果:SFI测量结果表明,神经损伤后2周,重度组(-100.042±4.667)完全丧失神经功能,中度组(-65.250±23.705)具有部分神经功能,轻度组(-26.792±18.762)神经功能完全恢复。重度组(t=200.900,P<0.0001)及中度组(t=4.830,P<0.05)与对照组比较差异均有统计学意义,轻度组与对照组比较差异无统计学意义(t=2.246,P>0.05)。CMAP测量发现重度组的神经传导被完全阻断,中度损伤组的动作电位在损伤后20 d前下降,并在损伤后20 d开始恢复。重度组(10.250±7.146)与对照组比较差异有统计学意义(t=198.900,P<0.01),中度组(39.875±20.259)与对照组比较(t=8.944,P<0.05),轻度组(86.125±9.094)与对照组比较无统计学意义(t=1.980,P>0.05)。滚轮运动实验结果轻度牵拉组(2.692±0.240),10 d后逐渐恢复正常,中度(1.891±0.328)及重度(0.804±0.280)恢复不明显。各组与对照组比较差异均有统计学意义(t=11.330、12.220、30.490,P<0.01)。热刺激实验发现损伤后对热反应明显降低并且重度组无恢复,各组与对照组比较差异均有统计学意义(t=3.623、8.994、215.749,P<0.05)。结论:以拉力为参数可以稳定地建立不同程度的坐骨神经牵拉性损伤动物模型。
Objective To establish an animal model of rat sciatic nerve injury(SNI)by traction and observe the sciatic nerve function index,electrophysiological and pathological changes after sciatic nerve traction.Methods Totally,24 SPF rats were randomly divided into 4 groups:control group(A),light(B),moderate(C),and severe(D)traction groups,with 6 rats in each group.The left sciatic nerve was exposed.The wound was closed after 10 min of exposure in group A,and the wound was closed after 10 min of pulling the sciatic nerve with different tensions in groups B,C and D.The sciatic nerve function index,Rotarod test and Hargreaves test,nerve target muscle compound muscle action potential,and sciatic nerve traction segment histopathology were observed.Results SFI measurement showed that 2 weeks after nerve injury,the severe group(-100.042±4.667)completely lost nerve function(t=200.900,P<0.01),the moderate group(-65.250±23.705)had partial nerve function(t=4.830,P<0.05),and the mild group(-26.792±18.762)had complete recovery of nerve function(t=2.246,P>0.05).CMAP measurement found that the nerve conduction in the severe group(10.250±7.146)was completely blocked(t=198.900,P<0.01),and the action potential of the moderate injury group(39.875±20.259)decreased in the first 20 days after the injury,and began to recover 20 days after the injury(t=8.944,P<0.05).No significant difference was found between mild group(86.125±9.094)and control group(t=1.980,P>0.05).The results of the Rotarod test gradually returned to normal after 10 days in the mild traction group(2.692±0.240),while the recovery in the moderate(1.891±0.328)and severe(0.804±0.280)groups was not obvious.There was significant difference between control group and experimental groups(t=11.330,12.220,30.490,P<0.05).Hargreaves test found that the thermal response was significantly reduced after injury and there was no recovery in the severe group.There was significant difference between control group and experimental groups(t=3.623,8.994,215.749,P<0.05).Conclusion Using te
