摘要
医学是除社会科学之外不确定性程度最高的知识领域。临床决策过程中,对医疗的确定性和不确定性的不同认识是医患之间出现分歧的一个重要原因。医患共同决策并不能减少医疗本质上的不确定性,但有助于患者对医疗的不确定性有更充分、具体的认识,从而帮助患者建立合理的预期、选择适宜的方案。医患共同决策对医患双方及医疗卫生体系提出了更高的要求。充分认识医疗的确定性和不确定性、提供合理的诊治策略和方法是医者的责任,而理解和接受医疗的不确定性也是患者应有的理性和担当。
Besides social science,medicine presents extremely prominent uncertainty.In clinical decision-making process,different understandings of the certainty and uncertainty of medicine is one of the causes of doctor-patient dissension.Shared decision-making can not reduce the uncertainty in the nature of medicine.However,it helps patients better understand the uncertainty of medicine so as to establish a reasonable anticipation and choose an appropriate strategy.It places higher demands for doctors,patients and health care system.It is the responsibility of the clinicians to fully understand the certainty and uncertainty of medicine and to provide reasonable strategy of diagnosis and treatment,while it is also the obligation of patients to understand and accept the uncertainty of medicine.
