摘要 目的分析Xp11.2易位/TFE3基因融合相关性肾癌(简称Xp11.2易位性肾癌)的MRI表现及特征。方法选取经手术病理证实的6例Xp11.2易位性肾癌患者资料,所有患者术前均行MRI检查,对肿瘤MRI特征进行分析。结果6例患者中5例年龄在25~38岁;6例T2WI均表现为不均匀低信号,2例表现为T1WI等信号;6例DWI信号均明显高于肾实质,ADC图病灶呈明显低信号;6例同反相位均未见脂质成分;1例无出血,5例不同程度出血;5例表现为中度强化,1例表现为明显强化,6例延迟期均廓清;6例均有假包膜,其中3例假包膜不完整,肾周癌灶形成或肾盂及输尿管受累。结论Xp11.2易位性肾癌具有一定的MRI信号特点,增强对该病的认识,有助于提高术前诊断的正确率。 Objective To analyze the imaging findings and imaging characteristics analysis of renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusions(abbreviation as Xp11.2 translocation renal cell carcinoma).Methods A retrospective analysis of 6 cases of Xp11.2 translocation renal cell carcinoma confirmed by operation and pathology was performed.All patients underwent MRI before operation.The MRI features of the tumor were analyzed.Results Among the 6 patients,5 cases were between 25 and 38 years old;T2WI signals of 6 cases were nonuniform low signals;T1WI signals of 2 cases were equal to the renal cortex;DWI signals of 6 cases were significantly higher than the renal parenchyma,and the ADC signals were significantly reduced;lipid signal of 6 cases was not observed in out phase and in phase;1 cases had no hemorrhage,5 cases had a different degree hemorrhage;5 cases showed moderate enhancement,1 case showed obvious enhancement,6 cases were washed out;6 cases had capsules,3 cases had incomplete capsule,with perirenal cancer formation or renal pelvis and ureteral involvement.Conclusion Renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusions has certain MRI signal characteristics,and improving the understanding of the disease will help to improve the preoperative diagnosis rate.
