摘要
目的探讨超声引导下腹横肌平面阻滞在妇科腹腔镜手术术后镇痛中的应用效果。方法选取2019年9月至2020年7月在我院行妇科腹腔镜手术的100例患者作为研究对象,将其随机分为对照组和观察组,各50例。对照组术后给予静脉自控镇痛泵镇痛,观察组在对照组基础上联合超声引导下腹横肌平面阻滞。比较两组的镇痛效果。结果观察组术后12、24 h的E、NE、COR水平低于对照组(P<0.05)。观察组术后不同时间点的VAS评分低于对照组(P<0.05)。观察组术后6 h的镇静评分优于对照组(P<0.05)。观察组术后首次镇痛泵按压时间晚于对照组,镇痛泵按压次数和舒芬太尼用量少于对照组(P<0.05)。观察组术后48 h的PSQI、GCQ评分优于对照组(P<0.05)。结论超声引导下腹横肌平面阻滞用于妇科腹腔镜手术术后镇痛中,可减轻患者的应激反应,缓解疼痛,提高舒适度及睡眠质量。
Objective To investigate the application effect of ultrasound-guided transversus abdominis plane block in postoperative analgesia of gynecological laparoscopic surgery.Methods A total of 100 patients who underwent gynecological laparoscopic surgery in our hospital from September 2019 to July 2020 were selected as the research objects and randomly divided into control group and observation group,with 50 cases in each group.The control group were given patient controlled intravenous analgesia after operation,and the observation group were given ultrasound-guided transversus abdominis plane block on the basis of the control group.The analgesia effects of the two groups were compared.Results The levels of E,NE and COR in the observation group at 12 and 24 h after operation were lower than those in the control group(P<0.05).The VAS scores of the observation group at different time points after operation were lower than those of the control group(P<0.05).The sedation score of the observation group at 6 h after operation was better than that of the control group(P<0.05).The first postoperative analgesia pump pressing time of the observation group was later than that of the control group,and the analgesia pump pressing times and sufentanil dosage were less than those of the control group(P<0.05).The PSQI and GCQ scores of the observation group at 48 h after operation were better than those of the control group(P<0.05).Conclusion Ultrasound-guided transversus abdominis plane block in postoperative analgesia of gynecological laparoscopic surgery can reduce stress response,relieve pain,improve comfort and sleep quality.
