摘要
目的分析盐酸氨溴索联合纤维支气管镜吸痰对老年肺炎的疗效及安全性。方法将我院2020年1月~2021年1月共128例老年肺炎患者设为研究对象,采用数字表随机法分为观察组和对照组,各64例。对照组给予纤维支气管镜吸痰进行治疗,观察组给予盐酸氨溴索联合纤维支气管镜吸痰进行治疗,比较两组治疗前后机体炎性因子水平[C反应蛋白(CRP)、白细胞计数(WBC)、降钙素原(PCT)],肺功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、第1秒用力呼气容积占用力肺活量百分比(FEV1%)],血气指标[动脉氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))、氧合指数(PaO_(2)/FiO_(2))]及预后情况,包括机械通气时间、抗生素使用时间、ICU时间。结果治疗后,两组CRP、WBC、PCT均低于治疗前,FEV1、FVC、FEV1%、PaO_(2)、SaO_(2)及PaO_(2)/FiO_(2)均高于治疗前,且观察组优于对照组,差异有统计学意义(P<0.05);观察组机械通气时间、抗生素使用时间以及ICU时间均低于对照组,差异有统计学意义(P<0.05)。结论盐酸氨溴索联合纤维支气管镜吸痰治疗老年肺炎具有良好的临床疗效,能有效缓解机体炎症水平,改善机体呼吸功能,促进肺部功能的恢复,且有利于缩短机械通气时间,提高治疗安全性,预后较好。
Objective To analyze the efficacy and safety of ambroxol hydrochloride combined with fiberoptic bronchoscopy for sputum suction in elderly patients with pneumonia.Methods A total of 128 elderly patients with pneumonia from January 2020 to January 2021 in our hospital were selected as the research objects.They were randomly divided into observation group and control group by number table,with 64 cases in each group.The control group was treated with fiberoptic bronchoscope suction,and the observation group was treated with ambroxol hydrochloride combined with fiberoptic bronchoscope suction.The levels of inflammatory factors[C-reactive protein(CRP),white blood cell count(WBC),procalcitonin(PCT)],lung function[forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC),the forced expiratory volume in the first second occupies the percentage of forced vital capacity(FEV1%)],blood gas indicators[arterial oxygen partial pressure(PaO_(2)),arterial blood oxygen saturation(SaO_(2)),oxygenation index(PaO_(2)/FiO_(2))]and prognosis,including mechanical ventilation time,antibiotic use time,ICU time.Results After treatment,CRP,WBC,and PCT in the two groups were lower than before treatment,FEV1,FVC,FEV1%,PaO_(2),SaO_(2) and PaO_(2)/FiO_(2) were all higher than before treatment,and the observation group was better than the control group,the difference was statistically significant(P<0.05);The mechanical ventilation time,antibiotic use time and ICU time in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion Ambroxol hydrochloride combined with fiberoptic bronchoscope sputum suction has a good clinical effect in the treatment of elderly pneumonia.It can effectively alleviate the level of body inflammation,improve the body's respiratory function,promote the recovery of lung function,and help shorten the time of mechanical ventilation,improve the safety of treatment,and have a better prognosis.
