摘要
目的旨在对上海市某三甲医院达芬奇机器人辅助与腹腔镜辅助前列腺癌手术进行医疗指标和卫生经济学评价,为临床医师、患者及家属选择前列腺癌术式提供参考。方法筛选出该院2019年1月1日至2020年11月30日的达芬奇机器人辅助手术和腹腔镜辅助手术病例,进行回顾性分析,对比两者的医疗安全、住院指标和经济费用等医疗和卫生经济学指标。用SPSS23.0统计软件进行数据统计对比分析。结果达芬奇机器人手术组和腹腔镜手术组两组患者术前特征均衡可比的情况下,前者的手术时间显著长于腹腔镜手术组,但是在术中出血量、住院时间、术后拔管时间、术后住院时间4个指标上均显著优于腹腔镜手术组。达芬奇机器人手术组的治疗费、护理费、手术总费用均显著低于腹腔镜手术组,而卫生材料费、住院总费用、自费部分均显著高于腹腔镜手术组。结论确认达芬奇机器人辅助手术的疗效与安全性,同时提示卫生经济学评价有助于改善达芬奇机器人辅助手术开展的规范和制度。
Objective To evaluate the medical factors and health economics of Da Vinci robot assisted and laparoscopic assisted prostate cancer surgery in a tertiary Class A hospital in Shanghai,so as to provide reference for clinicians,patients and their families to choose from prostate cancer surgery options.Methods The cases with Da Vinci robot assisted surgery or laparoscopic assisted surgery in the hospital from January 1,2019 to November 30,2020 were selected and analyzed retrospectively.The medical factors such as medical safety,hospitalization index and economic cost were compared.SPSS 32.0 statistical software was used for data analysis.Results The operation time of Da Vinci robot surgery group was significantly longer than that of laparoscopic surgery group,but the blood loss,hospital stay,extubation time and hospital stay of Da Vinci robot surgery group were significantly better than those of laparoscopic surgery group.The treatment cost,nursing cost and total operation cost of Da Vinci robot operation group were significantly lower than those of laparoscopic operation group,while the medical material cost,total hospitalization cost and own expense of Da Vinci robot operation group were significantly higher than those of laparoscopic operation group.Conclusion This study confirmed the efficacy and safety of Da Vinci robot assisted surgery,and suggested that health economics evaluation is helpful to improve the norms and regulations of this kind of surgery.
