摘要
目的分析益气调中法联合顺铂+紫杉醇(TP)化疗方案治疗中晚期食管癌的临床疗效和安全性。方法将83例中晚期食管癌患者按治疗方法的不同分为研究组(n=42)和对照组(n=41)。比较两组患者临床疗效、中医症候积分、生活质量及不良反应。结果研究组患者总有效率为95.24%(40/42),高于对照组的85.37%(35/41),差异有统计学意义(P﹤0.05)。研究组患者恶心呕吐、纳差乏力、腹泻及脘腹胀满中医症候积分均明显低于对照组,差异均有统计学意义(P﹤0.01)。治疗前,两组患者生命质量测定量表(QLQ-C30)及卡氏功能状态(KPS)评分比较,差异均无统计学意义(P﹥0.05);治疗后,研究组患者QLQ-C30及KPS评分均明显高于对照组,差异均有统计学意义(P﹤0.01)。研究组患者消化道不良反应(恶心呕吐、腹泻)较对照组轻,但差异均无统计学意义(P﹥0.05)。结论益气调中法可有效缓解TP化疗方案的部分不良反应,且安全性高,联合治疗对提高中晚期食管癌患者生活质量有明显优势,值得临床推广。
Objective To investigate the clinical effect and safety of the Yiqi Tiaozhong method combined with cisplatin and paclitaxel(TP)chemotherapy in the treatment of middle and advanced esophageal cancer.Method A total of 83 patients with middle and advanced esophageal cancer were divided into a study group(n=42)and a control group(n=41)according to the varying treatment regimens they received.The clinical efficacy,traditional Chinese medicine(TCM)syndrome weighted score,quality of life,and adverse events were compared between the two groups.Result The overall response rate of patients in the study group was 95.24%(40/42),which was higher than 85.37%(35/41)in the control group,the difference was statistically significant(P<0.05).The TCM symptom weighted scores of patients with nausea and vomiting,anorexia,diarrhea,and abdominal distension in the study group were significantly lower than those in the control group,the differences were statistically significant(P<0.01).Before treatment,there was no statistically significant difference between the two groups of patients on the scores of quality of life questionnaire core 30(QLQ-C30)and Karnofsky performance status(KPS)(P>0.05).After treatment,the QLQ-C30 and KPS scores of patients in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.01).The gastrointestinal events(nausea and vomiting,diarrhea)of the study group were milder than those of the control group,but the difference showed no statistically significant(P>0.05).Conclusion The Yiqi Tiaozhong method could effectively alleviate some of the adverse events of TP chemotherapy with reliable high safety.The combined treatment has apparent advantages in improving the quality of life of patients with middle and advanced esophageal cancer and is worthy of clinical promotion.
