摘要
目的对不同来源的沙门菌分离株的耐药状况进行监测,掌握吉林省沙门菌的耐药趋势和耐药谱,为指导抗生素在人类临床和动物饲养中的合理应用,控制耐药菌株的传播提供重要的参考性数据。方法近4年食品中检出的沙门菌和病人粪便中分离出的沙门菌,利用微量肉汤稀释法,用含有14种抗生素的药敏板进行耐药性最低抑菌浓度(MIC)检测。结果病人粪便标本中检出沙门菌216株,食品中检出62株。对278株沙门菌耐药性按样品的种类进行分别统计,病人粪便中检出的沙门菌总耐药率为95.8%(207/216)。多重耐药有160株,占74.1%(160/216)。食品中检出的沙门菌总耐药率67.7%(42/62),对头孢他啶、亚胺培南、头孢西丁、头孢噻肟、阿奇霉素是敏感的。多重耐药有14株,占22.6%(14/62)。结论本次监测病人粪便中检出的沙门菌存在多种抗生素的耐药,并且多重耐药严重,耐药谱多样。病人粪便中检出的沙门菌对8种抗生素的耐药率远高于食品样品中检出的沙门菌,两者差异有统计学意义(均P<0.05)。应高度重视和加强沙门菌的耐药性监测,以保证食品安全和人类健康。
Objective To monitor the drug resistance status of salmonella isolates from different sources, master the drug resistance trend and drug resistance spectrum of salmonella isolates in our province, and provide important reference data for guiding the rational application of antibiotics in human clinical and animal feeding, and controlling the spread of drug resistant strains.Methods The minimum antimicrobial resistance concentration(MIC) of salmonella detected in food of the past 4 years and in the feces of patients were determined by microbroth dilution method with 14 antibiotics.Results 216 strains of salmonella were detected in fecal samples and 62 strains in food samples. The drug resistance of 278 strains of salmonella was measured by type of sample. The total resistance rate of salmonella detected in the patient’s feces was 95.8%(207/216). There were 160 strains with multiple resistance, accounting for 74.1%(160/216). The total drug resistance rate of salmonella detected in food was 67.7%(42/62). These strains were sensitive to ceftadine, imipenem, cefoxime, cefotaxime and azithromycin. There were 14 strains with multiple resistance, accounting for 22.6%(14/62).Conclusion The salmonella bacteria detected in the feces of the patients were resistant to various antibiotics, and the multiple resistance was serious. The resistance rate of salmonella detected in patients’ feces to various antibiotics was much higher than that of salmonella detected in food samples, and the difference was statistically significant(P<0.05). To ensure food safety and human health, high attention should be paid to the surveillance of salmonella resistance.
