摘要
目的研究非小细胞肺癌患者凝血功能与肿瘤转移复发的相关性。方法选择2018年4月至2019年4月我院收治的非小细胞肺癌患者90例为观察对象,统计患者各项凝血功能指标水平变化,分析其与肿瘤转移复发的相关性。结果相较于良性肺部疾病,NSCLC患者的PLT、APTT差异无统计学意义(P>0.05);而PT出现明显降低,FIB、D-D指标出现明显升高(P<0.05)。非小细胞肺癌患者行外科手术治疗后发生肿瘤转移者65例,未发生转移者25例;肿瘤复发者58例,未复发者32例。转移组APTT水平低于未转移组,D-D、FIB水平则相比未转移组提高,差异有统计学意义(P<0.05);而两组PLT、PT指标水平差异无统计学意义(P>0.05)。肿瘤复发组D-D、FIB水平对比未复发组提高(P<0.05),组间PLT、PT、APTT指标水平差异无统计学意义(P>0.05)。经Logistic回归分析,凝血指标APTT、D-D、FIB水平变化与非小细胞肺癌患者肿瘤转移复发有相关性。结论非小细胞肺癌治疗后肿瘤转移复发与患者凝血功能指标APTT、D-D、FIB水平变化密切相关,可将其作为预后与诊疗进行的参考指标。
Objective To study the correlation between coagulation function and metastasis and recurrence of NSCLC.Methods 90 patients with NSCLC admitted to our hospital from April,2018 to April,2019 were selected as the observation objects,and the changes of coagulation function indexes were statistically analyzed for their correlation with tumor metastasis and recurrence.Results Compared with patients with benign lung disease and NSCLC,PLT and APTT of NSCLC patients had no statistical difference(P>0.05).PT decreased significantly,while FIB and D-D increased significantly(P<0.05).There were 65 cases with metastasis and 25 cases without metastasis,also 58 cases with recurrence and 32 cases without recurrence.The level of APTT in the metastasis group was lower than that in the non-metastasis group,while the level of D-D and Fib in the metastasis group was higher than that in the non-metastasis group(P<0.05),and the level of PLT and PT in two groups had no significant difference(P>0.05).The levels of D-D and Fib in the recurrence group were higher than those in the non-recurrence group(P<0.05).There was no significant difference in PLT,PT and APTT between the two groups(P>0.05).Logistic regression analysis showed that the levels of APTT and APTT were correlated with the recurrence of non-small cell lung cancer.Conclusion Metastasis and recurrence of NSCLC after treatment are closely related to the changes of APTT,D-D and FIB,which can be used as a reference index for prognosis and diagnosis.
