摘要
目的探讨晚期肺癌炎症指数(ALI)对肺癌程序性死亡受体配体1(PD-L1)表达的预测价值。方法回顾性分析2018年12月~2020年6月收治的116例诊断为肺癌患者的资料,根据PD-L1表达是否≥1%分为2组,收集患者初诊时的血常规、血清白蛋白(Alb)、身体质量指数(BMI),计算ALI数值,采用免疫组化的方法获取PD-L1表达结果;其中57例患者为腺癌且有表皮生长因子受体(EGFR)基因检测结果,利用SPSS22.0进行数据统计分析。结果两组患者年龄、BMI、中性粒细胞数、淋巴细胞数、血清白蛋白、ALI等数据比较,差异无统计学意义(P>0.05)。ROC曲线分析结果显示ALI对PD-L1表达预测的曲线下面积(AUC)为0.513。EGFR基因突变与PD-L1表达水平的皮尔逊相关系数r值为0.167。结论 ALI作为肺癌患者PD-L1表达的预测因子价值较低,但EGFR突变可促进PD-L1的表达。
Objective To explore the predictive value of advanced lung cancer inflammation index(ALI) on the expression of lung cancer programmed death receptor ligand 1(PD-L1).Methods A retrospective study was employed for study.The data of 116 patients were confirmed lung cancer admitted to the Affiliated Hospital of Weifang Medical University from December 2018 to June 2020.According to whether the expression of PD-L1 was greater than or equal to 1%,they were divided into two groups.Such information includes blood routine, serum albumin(Alb),and body mass index(BMI) of the patients were collected at the time of initial diagnosis, and the ALI value was calculated.The results of PD-L1 expression were assessed by immunohistochemistry.Among them, 57 patients were adenocarcinoma and had epidermal growth factor receptor gene test results, and the data were analyzed by SPSS22.0 statistical software.Results There was no statistically significant difference between the two groups of patients’ age, BMI,neutrophil count, lymphocyte count, serum albumin, ALI and other data(P>0.05).ROC curve analysis results showed that the area under the curve(AUC) predicted by ALI for PD-L1 expression was 0.513.The Pearson correlation coefficient between EGFR mutation and PD-L1 expression level was 0.167.Conclusion ALI has low value as a predictor of PD-L1 expression in lung cancer patients, but EGFR mutations can promote the expression of PD-L1.
