摘要
目的分析BRAF V600E基因突变与甲状腺乳头状癌(PTC)及其临床病理特征的关系。方法选取2013年1月~2018年12月病理诊断为PTC的患者337例及甲状腺组织正常者50例,分析年龄、性别、肿瘤大小、病灶双侧性和多灶性、淋巴结转移情况和腺外侵犯等临床特征。应用聚合酶链式反应(PCR)技术进行BRAF V600E基因状态检测,分析BRAF V600E突变与PTC临床病理学的相关性。结果 BRAF V600E突变在61.7%(208/337)PTC患者中发现,突变未在正常甲状腺组织中发现。BRAF突变型的PTC相对于野生型的腺外侵犯(40.9%)和淋巴结转移(38.9%)比例更高,差异有统计学意义(P<0.05)。BRAF V600E突变中女性患者居多,占76.0%,40~65岁区间的患者发病最多,占44%,肿瘤平均直径大小为1.56cm,双侧性和多灶性分别占15.1%和43.0%,伴发结节性甲状腺肿和桥本氏甲状腺炎比率分别为60.8%和32.9%,但差异均无统计学意义(P>0.05)。结论 BRAF V600E突变发生在PTC病灶中,突变型PTC患者腺外侵犯和淋巴结转移比例更高,显示出突变可能与肿瘤的高侵袭性相关。
Objective To analyze the relationship between the mutation of BRAF V600 E gene and the clinicopathological features of papillary thyroid carcinoma(PTC).Methods From January 2013 to December 2018,337 cases of PTC and 50 cases of normal thyroid tissue were pathologically diagnosed in Weifang Hospital of Traditional Chinese Medicine.The clinicopathological features such as age, sex, tumor size, bilateral and multifocal lesions, lymph node metastasis and extraglandular invasion were analyzed.The gene status of BRAF V600 E was detected by polymerase chain reaction(PCR),and the correlation between BRAF V600 E mutation and clinicopathology of PTC was further analyzed.Results BRAF V600 E mutations were found in 61.7%(208 of 337) patients, but not in normal thyroid tissues.The proportion of PTC of BRAF mutant was higher than that of wild type in extraglandular invasion(40.9%) and lymph node metastasis(38.9%).Most of the BRAF V600 E mutations were female, accounting for 76.0% of the patients aged 40 to 65,accounting for 44%,the average diameter of the tumor was 1.56 cm.Bilateral and multifocal accounted for 15.1% and 43.0%,respectively.The incidences of nodular goiter and HT were 60.8% and 32.9%,respectively, but there was no statistical difference(P>0.05).Conclusion BRAF V600 E mutation occurred in PTC lesions.Patients with mutant PTC have a higher proportion of extraglandular invasion and lymph node metastasis, indicating that the mutation may be related to the high invasiveness of the tumor.
