摘要
目的探讨热塑性聚氨酯(TPU)支架在机器人导航辅助经皮螺钉内固定术治疗腕舟骨骨折中的应用价值。方法2020年1月至2021年1月贵阳市第四人民医院骨三科应用机器人导航辅助经皮螺钉内固定术治疗腕舟骨骨折2例。患者均为男性,年龄分别为35岁和46岁;右、左侧各1例;骨折均为AO分型B1型,均为交通事故致伤。术中采用TPU支架固定患者患肢前臂、手和腕关节,以避免术中患肢移位。结果2例手术用时均为45 min。2例患者术后螺钉均位于舟骨轴心,实现有效固定;实际导针与规划导针成角分别为2.23°和1.68°,重合度较好;2例手术均一次完成打入导针,术中未需要调整;术中未使用止血带,均出血量<5 ml。结论使用TPU支架可以实现个体化腕关节固定,对应用机器人导航经皮螺钉内固定治疗腕舟骨骨折有辅助作用。
Objective To investigate the application value of thermoplastic polyurethane(TPU)brace in robot-assisted percutaneous screw fixation for scaphoid fractures of wrist.Methods Tow patients with scaphoid fracture of wrist underwent robot-assisted percutaneous screw fixation in the Third Department of Orthopedics,the Fourth People's Hospital of Guiyang From January 2020 to January 2021 were included.Both patients were male,aged 35 and 46,respectively.One patient was injured on the right side and the other one was injured on the left side.Both fractures were type B1 according to AO classification and were caused by traffic accident.TPU braces were used to secure the affected wrist to prevent movement.Results Two operations were performed in 45 min.The two screws were located in the axis of scaphoid and effective fixations were obtained.The angle between the actual guide needle and the planned guide needle was 2.23°and 1.68°,respectively.In both cases,the insertion of the guide needle was performed at one time without adjustment.No tourniquet was used during the operation and the amount of bleeding was less than 5 ml in the 2 cases.Conclusion Application of the TPU brace could achieve personalized wrist joint fixation and has a good auxiliary effect on robot-assisted percutaneous screw fixation in the treatment of scaphoid fracture of wrist.
