摘要
目的探讨度洛西汀和米氮平治疗老年抑郁症的临床疗效。方法选择我院于2017年8月至2018年12月收治的100例老年抑郁症患者,随机分组。米氮平治疗组老年抑郁症患者采取米氮平治疗缓解症状,度洛西汀治疗组老年抑郁症患者则采用度洛西汀治疗。比较两组老年抑郁症疗效;不良反应量表(TESS)评分以及药物起效时间;治疗前后患者汉密尔顿抑郁量表(HAMD)评分、躯体疼痛VAS评分、匹兹堡睡眠质量评分。结果度洛西汀治疗组的疗效和米氮平治疗组无显著差异,P>0.05。两组治疗后HAMD评分、躯体疼痛VAS评分、匹兹堡睡眠质量评分均有所改善,且两组HAMD评分改善的幅度相似,P>0.05;与治疗前相比,米氮平治疗组的匹兹堡睡眠质量评分降低更显著,度洛西汀治疗组的躯体疼痛VAS评分降低更显著,P<0.05。度洛西汀治疗组TESS评分以及药物起效时间和米氮平治疗组无显著差异,P>0.05。结论老年抑郁症患者应用度洛西汀与米氮平治疗的效果确切,二者均有效,可缓解老年抑郁症状,且安全性相似,但度洛西汀减轻疼痛的效果更好,而米氮平改善睡眠质量的效果更好。
Objective To explore the clinical efficacy of duloxetine and mirtazapine in the treatment of senile depression.Methods One hundred cases of elderly depression patients admitted to our hospital from August 2017 to December 2018 were selected and randomly divided into groups.Elderly depression patients in the mirtazapine treatment group were treated with mirtazapine to relieve their symptoms,and the duloxetine treatment group was treated with duloxetine in elderly depression patients.The efficacy of the two groups of elderly depression were compared;the side effects scale(TESS)score and the onset time of the drug;the Hamilton Depression Scale(HAMD)score,the body pain VAS score,and the Pittsburgh sleep quality score before and after treatment.Results The efficacy of the duloxetine treatment group was no significantly different from that of the mirtazapine treatment group,P>0.05.After treatment,the HAMD score,body pain VAS score,and Pittsburgh sleep quality score improved in the two groups,and the HAMD scores of the two groups improved similarly,P>0.05.Compared with before treatment,the Pittsburgh sleep quality score of the mirtazapine treatment group decreased more significantly;compared with before treatment,the VAS score of somatic pain in the duloxetine treatment group decreased more significantly,P<0.05.There was no significant difference in TESS score and drug onset time between duloxetine treatment group and mirtazapine treatment group,P>0.05.Conclusion The effect of duloxetine and mirtazapine in the treatment of elderly depression patients is definite.Both are effective and can relieve the symptoms of depression in the elderly with similar safety.However,duloxetine has a better effect on reducing pain,while mirtazapine has a better effect on improving sleep quality.
