摘要
目的:探讨厄贝沙坦联合贝前列素钠对老年高血压合并早期肾损害的临床疗效及其对患者血清炎症因子的影响。方法:选择82例老年高血压合并早期肾损害患者为研究对象,按治疗方式不同分为对照组和观察组,每组各41例。对照组患者接受厄贝沙坦治疗,观察组患者接受厄贝沙坦联合贝前列素钠治疗,疗程均为8周。比较两组患者治疗前后血压及血清肌酐(Scr)、胱抑素C(CysC)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、尿白蛋白排泄率(UAER)的表达水平。结果:治疗后,两组患者SBP、DBP均较治疗前降低,且观察组降低得更加明显(P<0.05);治疗后,两组患者得CysC及UAER水平均降低,且观察组均低于对照组(P<0.05);治疗前后,Scr水平在两组比较,差异无统计学意义(P>0.05);治疗后,血清TNF-α、IL-6水平在两组均降低,且观察组降低得更加明显(P<0.05)。结论:厄贝沙坦联合贝前列素钠治疗老年高血压早期肾损害可降低血压及血清TNF-α、IL-6水平,减少尿蛋白,保护患者的肾功能。
Objective:To investigate the clinical effect of irbesartan combined with beprost sodium in the treatment of elderly hypertension with early renal damage and its influence on serum inflammatory factors.Methods:82 cases of elderly patients with hypertension complicated with early renal damage were selected and divided into control group and observation group according to different treatment methods,41 cases in each group.The control group was treated with irbesartan,and the observation group was further treated with beraprost sodium.The course of treatment was 8 weeks.The blood pressure,serum creatinine(Scr)and Cystatin C(CysC),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and Urinary albumin excretion rate(UAER)of the two groups before and after treatment were compared.Results:After treatment,SBP and DBP of the two groups were significantly decreased,and the decrease of the observation group was more obvious(P<0.05).After treatment,CysC and UAER in the two groups were significantly lower,and the observation group were significantly lower than the control group(P<0.05).Before and after treatment,there was no significant difference in SCR level between the two groups(P>0.05).After treatment,serum TNF-αand IL-6 in the two groups were significantly lower,and the observation group decreased more significantly(P<0.05).Conclusion:Irbesartan combined with beprost sodium in the treatment of elderly hypertensive patients with early renal damage can effectively reduce blood pressure and serum TNF-αand IL-6,reduce urinary protein and protect renal function.
