摘要
目的探讨唱音音域(voice range profile,VRP)和话音音域(speech rande profile,SRP)对于声带息肉患者术后嗓音功能恢复评估的临床价值及两种音域图的相关性。方法以2018年2月至2019年9月行手术治疗的声带息肉患者28例为研究对象(声带息肉组,年龄25~49岁),以未经嗓音训练的正常嗓音者30例为对照组(年龄18~50岁),分别于声带息肉组手术前、术后1个月及正常组进行VRP和SRP测试,对VRP及SRP的最低频率、最高频率、音调范围、最小声压级、最大声压级、声压级范围等6个指标进行分析,比较声带息肉组手术前后的变化并与对照组比较,比较两种音域的相关性。结果与对照组相比,声带息肉组VRP与SRP平均最高频率、最大声压级、音调范围、声压级范围减小,平均最低频率及最小声压级增高(均为P<0.05),音域变窄。声带息肉组术后1个月VRP结果与术前相比,最高频率及最大声压级增大、最小频率及最小声压级减小、音调范围及声压级范围增大,差异均有统计学意义(P<0.05);声带息肉组术后1个月SRP结果与术前相比,最高频率及最大声压级增大、最小声压级减小、音调范围及声压级范围增大,差异有统计学意义(P<0.05),而最低频率均值减小,但差异无统计学意义(P=0.11)。声带息肉患者手术前后VRP与SRP的6个指标均显著正相关(0<r<1)。结论VRP与SRP作为嗓音声学音域值的量化指标,均能很好地评估声带息肉患者术后的发声功能;在评估声带息肉患者术后嗓音音域的恢复方面,VRP的敏感性高于SRP。
Objective To study the clinical value of voice range profile(VRP)and speech range profile(SRP)in the evaluation of voice function recovery in patients with vocal cord polyps after operation,and to study the correlation between the two register maps.Methods From February 2018 to September 2019,28 patients underwent vocal cord polyp surgery in our department,and 30 patients with normal voice without voice training were included in this study.The voice range of vocal cord polyps before operation,1 month after operation and normal group were measured respectively.The minimum frequency,maximum frequency,tone range,minimum sound pressure level,maximum sound pressure level and sound pressure level range of VRP and SRP were measured.The changes of the parameters before and after the operation and the correlation between the two ranges were analyzed and compared.Results Both VRP and SRP showed that the average maximum frequency,maximum sound pressure level,tone range and sound pressure level range of vocal cord polyp group were lower than those of control group,while the average minimum frequency and minimum sound pressure level were increased(P<0.05)and the range of voice became narrower.VRP results the highest frequency and the highest sound pressure level increased,the minimum frequency and minimum sound pressure level decreased,the tone range and sound pressure level range increased after 1 month compared with those before operation,and the differences were statistically significant(P<0.05).SRP resultsthe highest frequency and the highest sound pressure level increased and the minimum sound pressure level increased 1 month after operation.There were significant differences among the five parameters(P<0.05),but the mean of the lowest frequency decreased Pearson correlation coefficient(R)(P=0.11>0.05).The six parameters of VRP and SRP had significant positive correlation(0<r<1)before and after operation in patients with vocal cord polyps.Conclusion As a quantitative index of acoustic range value,VRP and SRP can evaluate
