MRI对于肿块强化和非肿块强化乳腺癌病灶新辅助化疗后肿瘤回缩方式的初步探讨 被引量:1

摘要 目的:探讨MRI对于肿块强化和非肿块强化乳腺癌病灶新辅助化疗后肿瘤回缩方式的表现及其病理基础,以及对外科手术方案选择的临床意义。方法:回顾性分析自2018年1月—2019年10月之间70例局部进展期乳腺癌患者的临床、MRI及病理资料,所有患者均接受新辅助化疗,且满足化疗前、后均行MRI检查。MRI检查包括动态增强,最后获得新辅助化疗前后肿瘤的MRI增强方式和肿瘤的缩小模式。结果:在动态增强图像上将病灶分为肿块强化组(34例)及非肿块强化组(36例),MRI退缩方式分为向心型(31例)、树枝型(17例)和混合型(22例)。两种MRI强化方式与MRI退缩方式之间的差异有统计学意义,χ^(2)=47.043,P=0.000,肿块强化以向心型回缩为主,占向心型回缩的93.5%(29/31),明显高于非肿块强化组。非肿块强化以树枝型和混合型为主,分别占其回缩方式比例为100%和77.3%,明显高于肿块强化。结论:MRI强化方式是预判乳腺癌肿瘤病理回缩方式的有效检查手段,这对于患者和外科医生在后续治疗中获得合理的预期具有重要意义。 Objective: To investigate the clinical significance of MRI in tumor retraction after neoadjuvant chemotherapy for breast cancer with mass like and non-mass like enhancement. Methods: The clinical, MRI and pathological data of 70 patients with locally advanced breast cancer from January 2018 to October 2019 were analyzed retrospectively. All patients received neoadjuvant chemotherapy, and MR examination was performed before and after chemotherapy. Finally, MRI enhancement mode and tumor retraction mode before and after neoadjuvant chemotherapy were obtained. Results: On dynamic contrast-enhanced images, the lesions were divided into mass like enhancement group(34 cases) and non-mass like enhancement group(36 cases). MRI retreat mode was divided into centripetal type(31 cases), dendritic type(17 cases) and mixed type(22 cases). The difference between the two kinds of MRI enhancement and MRI retraction was statistically significant, χ^(2)=47.043,P=0.000, and the mass like enhancement was mainly retracted to the heart, accounting for 93.5%(29/31) of them. It was significantly higher than that of the non-mass like enhancement group, and the non-mass like enhancement mainly consisted of branch type and mixed type, accounting for 100% and 77.3% respectively, which were significantly higher than that of the mass like enhancement. Conclusion: MRI enhancement mode and retraction mode are effective methods to predict the pathological retraction mode of breast cancer, which is of great significance for patients and surgeons to obtain reasonable expectations in the follow-up treatment.
