摘要
目的研究地屈孕酮联合孕三烯酮用于手术治疗的子宫内膜异位症中的效果及对复发率的影响。方法 98例接受腹腔镜手术治疗的子宫内膜异位症患者,按照随机数字表法分为对照组和观察组,各49例。对照组术后采用孕三烯酮治疗,观察组在对照组基础上联合地屈孕酮治疗。比较两组疗效、治疗前后临床指标水平及视觉模拟评分法(VAS)评分、复发率。结果观察组治疗总有效率为95.92%,高于对照组的81.63%,差异有统计学意义(P<0.05)。治疗后,两组血清雌二醇(E2)、促卵泡生成素(FSH)、糖类抗原125(CA-125)水平均较治疗前下降,且观察组低于对照组,差异有统计学意义(P<0.05)。两组治疗后黄体生成素(LH)水平与治疗前比较及组间比较,差异均无统计学意义(P>0.05)。治疗后,两组痛经、盆腔痛、性交痛的VAS评分均较治疗前下降,且观察组痛经(0.10±0.01)分、盆腔痛(0.14±0.03)分、性交痛(0.11±0.01)分,均低于对照组的(0.34±0.06)、(0.39±0.05)、(0.42±0.02)分,差异有统计学意义(P<0.05)。观察组复发率4.08%,低于对照组的16.33%,差异有统计学意义(P<0.05)。结论孕三烯酮联合地屈孕酮可有效改善患者疼痛症状,调节体内性激素水平,降低血清CA-125水平,从而增强疗效,并能显著降低患者复发率。
Objective To study the effect of dydrogesterone combined with gestrinone in the postoperative treatment of endometriosis and its influence on recurrence rate.Methods A total of 98 cases of endometriosis patients undergoing laparoscopic surgery were divided into control group and observation group according to random numerical table,with 49 cases in each group.The control group was treated with gestrinone,and the observation group was treated with dydrogesterone on the basis of the control group.The clinical efficacy,clinical indicators and visual analogue scale(VAS)score before and after treatment,and recurrence rate were compared between the two groups.Results The total effective rate of the observation group was 95.92%,which was higher than 81.63%of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of serum estradiol(E2),follicle stimulating hormone(FSH),carbohydrate antigen 125(CA-125)of the two groups were lower than those before treatment,and the observation group was lower than the control group.All the difference was statistically significant(P<0.05).After treatment,there was no statistically significant difference in luteinizing hormone(LH)level between the two groups and compared with those before treatment(P>0.05).After treatment,the VAS score of dysmenorrhea,pelvic pain and intercourse pain of the two groups were lower than those before treatment,and those scores of the observation group[(0.10±0.01),(0.14±0.03),(0.11±0.01)points]were lower than the control group[(0.34±0.06),(0.39±0.05),(0.42±0.02)points].All the difference was statistically significant(P<0.05).The recurrence rate of the observation group was 4.08%,which was lower than 16.33%of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of gestrinone and dydrogesterone can effectively improve the pain symptoms,regulate the level of sex hormones,reduce the level of serum CA-125,so as to enhance the efficacy,and significantly reduce the recurre
