摘要
目的探讨无创呼吸机辅助通气治疗重症哮喘的效果。方法 120例重症哮喘患者,随机分为对照组与研究组,各60例。对照组仅采取对症支持治疗,研究组在对照组治疗基础上采取无创呼吸机辅助通气治疗。比较两组患者动脉血气分析指标[pH值、动脉血氧分压(PaO_(2))、动脉二氧化碳分压(PaCO_(2))、氧合指数(PaO_(2)/FiO_(2))]临床症状体征消失时间、临床疗效。结果治疗后,研究组PaO_(2)、PaCO_(2)、PaO_(2)/FiO_(2)分别为(77.41±5.39)、(35.47±1.04)、(279.83±17.16)mm Hg(1 mm Hg=0.133 kPa),均明显优于对照组的(72.63±6.18)、(38.90±1.57)、(221.07±19.42)mm Hg,差异具有统计学意义(P<0.05)。两组p H值比较,差异无统计学意义(P>0.05)。研究组哮鸣音消失时间为(3.48±0.93)d、气促消失时间为(2.14±0.77)d、喘息消失时间为(2.35±0.83)d,均短于对照组的(5.72±0.53)、(4.17±0.62)、(4.03±0.71)d,差异具有统计学意义(P<0.05)。研究组治疗总有效率为93.3%,高于对照组的80.0%,差异具有统计学意义(P<0.05)。结论无创呼吸机辅助通气对重症哮喘的治疗效果显著,可有效改善患者的动脉血气分析指标,加快病情康复。
Objective To discuss the effect of non-invasive ventilator assisted ventilation in treating severe asthma.Methods A total of 120 patients with severe asthma were randomly divided into control group and research group,with 60 cases in each group.The control group received symptomatic and supportive treatment,and the research group received non-invasive ventilator assisted ventilation based on the control group.The arterial blood gas analysis indexes[pH,arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),oxygen sum index(PaO_(2)/FiO_(2))],disappearance time of clinical symptoms and signs,and clinical efficacy were compared between the two groups.Results After treatment,the levels of PaO_(2),PaCO_(2),PaO_(2)/FiO_(2) of the research group were(77.41±5.39),(35.47±1.04),(279.83±17.16)mm Hg(1 mm Hg=0.133 kPa),which were obviously better than(72.63±6.18),(38.90±1.57),(221.07±19.42)mm Hg of the control group,and the difference was statistically significant(P<0.05).The disappearance time of wheezing rale,shortness of breath,gasp of the research group were(3.48±0.93),(2.14±0.77),(2.35±0.83)d,which were shorter than(5.72±0.53),(4.17±0.62),(4.03±0.71)d of the control group,and the difference was statistically significant(P<0.05).The total effective rate of the research group was 93.3%,which was higher than 80.0%of the control group,and the difference was statistically significant(P<0.05).Conclusion Non-invasive ventilator has remarkable effect in the treatment of severe asthma,and it can effectively improve the arterial blood gas analysis index of patients and speed up the recovery.
