摘要
目的:分析不同剂量地佐辛,超前应用于内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)镇痛的效果及安全性。方法:择期行ESD的患者80例,美国麻醉医师协会(American Society of Anesthesiologists,ASA)Ⅰ~Ⅱ级,随机均分成4组:C组、D_(1)组、D_(2)组、D_(3)组。C组为对照组,D_(1)、D_(2)、D_(3)组分别于内镜置入前10 min,静脉注射地佐辛0.05、0.15、0.25 mg/kg。记录手术时间、术后拔管时间及各时间点的疼痛视觉模拟评分(visual analogue scale,VAS)、Ramsay镇静评分,同时观察不良反应发生情况。结果:D_(2)、D_(3)组的拔管时间均长于C、D_(1)组,D_(3)组的拔管时间长于D_(2)组(P<0.05);D_(2)、D_(3)组在术后各个时间点的VAS低于C、D_(1)组(P<0.05);D_(3)组在术后各个时间点的Ramsay高于D_(1)、D_(2)组(P<0.05);D_(3)组术后恶心呕吐的发生率高于C组,D_(2)、D_(3)组术后使用镇痛药的发生率低于C、D_(1)组(P<0.05)。结论:超前使用0.15 mg/kg地佐辛能有效减轻ESD术后疼痛,且不增加不良反应。
Objective:To analyze the efficacy and safety of the dose of dezocine in preemptive analgesia in endoscopic submucosal dissection(ESD).Methods:Eighty cases of American Society of Amesthesiologists(ASA)Ⅰ-Ⅱpatients underwent ESD were averagely divided into four groups:control(Group C)and dezocine 0.05,0.15 and 0.25 mg/kg(Group D_(1),D_(2),D_(3))injected via venous ten minutes before the operation.The time of operation,postoperational extubation,visual analogue scale(VAS),Ramsay sedation score at preset time,and the occurrence of complications were recorded.Results:The time of postoperational extubation in Group D_(2) and D_(3) was longer than in Group C and D_(1) and the time of postoperational extubation in Group D_(3) was longer than in Group D_(2)(P<0.05);the scores of VAS in Group D_(2) and D_(3) were lower than those in Group C and D_(1) at all preset time points;the scores of Ramsay in Group D_(3) were higher than in Group D_(1) and D_(2) at all preset time points(P<0.05);the incidence of nausea and vomiting in Group D_(3) was higher than in Group C,the incidence of narcotic requirement postoperation in Group D_(2) and D_(3) was lower than in Group C and D_(1)(P<0.05).Conclusion:Dezocine with the dose of 0.15 mg/kg in preemptive analgesia in ESD could relieve the pain after the surgery effectively with the lower incidence of adverse reactions.
