摘要
目的基于中医传承辅助平台V2.5分析国医大师王琦治疗慢性前列腺炎的用药规律。方法收集2016年9月-2019年9月王琦在北京中医药大学国医堂门诊治疗慢性前列腺炎的病案,将符合纳入标准的病案录入中医传承辅助平台V2.5,利用频次统计功能及关联规则分析、复杂系统熵聚类分析、无监督熵层次聚类分析对用药规律进行分析。结果共纳入慢性前列腺炎病案65个,涉及药物178味,处方122首。药物药性以寒、温、平为主,药味以苦、甘、辛为主,归经以肝经为主,其次为胃、脾、心经。药物频次按降序排列,前8味中药与当归贝母苦参丸高度相似,排名第9~16味的中药与复元活血汤高度相似。药物类别统计显示,前6类分别为活血调经类、利尿通淋类、化瘀止血类、清热燥湿类、理气类、补血类。关联规则分析显示,核心药物组合多由当归贝母苦参丸合复元活血汤药物加减,具有清热祛湿排浊、化瘀通络止痛之功,能够治疗湿热瘀浊阻滞前列腺、精窍不畅等症。聚类分析得出24个新核心药物组合、12首药物新方。结论王琦治疗慢性前列腺炎以湿热瘀浊阻滞下焦为主要病机,运用主病主方思想,以当归贝母苦参丸、复元活血汤为主方加减,对该病进行分期论治。
Objective To analyze the medication rules of TCM master Wang Qi in the treatment of chronic prostatitis based on the TCM Inheritance Support System V2.5.Methods Medical records of Wang Qi in treating chronic prostatitis in the outpatient department of Guoyi Hall of Beijing University of Chinese Medicine from September 2016 to September 2019 were collected and sorted out.The medical records that meet the inclusion criteria into the TCM Inheritance Support System V2.5 were checked and entered.The frequency statistical function included in the software and the analysis of association rules,complex system entropy clustering analysis,and unsupervised entropy hierarchical clustering analysis were used to analyze the medication rules.Results Totally 65 cases of chronic prostatitis were included,involving 178 kinds of Chinese materia medica and 122 prescriptions.The properties were mainly cold,warm and peaceful,and the flavors were mainly bitter,sweet and spicy.The main meridian was liver meridian,followed by stomach,spleen and heart meridians.According to the descending order of medicine frequency,it was found that the medicine composition of the top 8 kinds of Chinese materia medica was highly similar to that of Danggui Beimu Kushen Pills,and the medicine composition of the 9th to 16th Chinese materia medica was highly similar to that of Fuyuan Huoxue Decoction.According to the statistics of medicine categories,the first six categories were promoting blood circulation and regulating menstruation,diuresis and purging lymph,removing blood stasis and hemostasis,clearing heat and drying dampness,regulating qi and replenishing blood.Association rule analysis found that most of the core medicinal combinations were obtained from Danggui Beimu Kushen Pills and Fuyuan Huoxue Decoction,which had the effects of clearing heat,removing dampness and turbidity,removing blood stasis and unblocking meridians and relieving pain,and could treat such diseases as dampness heat and blood stasis blocking prostate and obstruction of essence o
