摘要
目的:探讨血清促甲状腺激素(thyroid stimulating hormone, TSH)与超声造影(contrast-enhanced ultrasound, CEUS)联合评分法在甲状腺超声影像报告和数据系统(thyroid imaging reporting and data system, TI-RADS)4类微小结节良恶性诊断中的应用价值。方法:回顾性分析300个TI-RADS 4类甲状腺微小结节(最大径≤1.0 cm)的血清TSH水平及超声造影特征,分别对每个结节进行赋值评分,与病理检查结果进行对照,构建ROC曲线,比较TSH法、CEUS法及二者联合评分法对TI-RADS 4类微小结节良恶性的诊断价值。结果:根据ROC曲线分析,联合评分法以6分作为诊断截点,敏感性与特异性分别为77.34%、81.40%,AUC值为0.867(95%CI:0.823~0.903)。联合法诊断效能高于TSH法和CEUS法,差异具有统计学意义(P值均<0.01)。结论:TSH与CEUS联合评分法对TI-RADS 4类甲状腺微小结节良恶性诊断具有较高的价值,值得临床推广应用。
Objective:To investigate the value of combined scoring method by serum thyroid stimulating hormone(TSH)and contrast-enhanced ultrasound(CEUS)in the diagnosis of thyroid imaging reporting and data system(TI-RADS)4 type micro-nodules.Methods:The TSH level and CEUS features of 300 TI-RADS 4 type micro-nodules were analyzed retrospectively.The scores of each nodule were evaluated and compared with pathological results.ROC curve was drawn to compare the diagnostic valve of TSH level,CEUS and the combined application for TI-RADS 4 type micro-nodules.Results:According to the ROC curve,when the cut-off value of combine scoring was 6,the sensitivity and specificity were 77.34%,81.40%,and AUC was 0.867(95%CI:0.823~0.903).The AUC value of TSH combined with CEUS was higher than that of single diagnosis,and the difference was statistically significant(all P<0.01).Conclusion:The combined scoring method has a high diagnostic value for benign and malignant TI-RADS 4 type thyroid micro-nodules.It was worthy of clinical application.
