摘要
目的探究鼻渊汤辨证加减对慢性鼻-鼻窦炎患者鼻黏膜清除功能及血清总免疫球蛋白E(Total immunoglobulin E,TIg E)、嗜酸细胞阳离子蛋白(Eosinophil cationicprotein,ECP)的影响。方法选取2018年2月—2019年12月中国医科大学附属第一医院收治的160例慢性鼻-鼻窦炎患者,按照随机数字表法分为对照组和观察组,各80例。对照组给予阿莫西林胶囊治疗,观察组给予鼻渊汤辨证加减治疗,2组均治疗3个月。比较2组治疗3个月后的临床疗效及并发症发生率。比较2组治疗前、治疗1个月后、治疗3个月后鼻黏膜清除功能,治疗前和治疗3个月后血清TIg E、ECP水平。结果观察组的临床总有效率为92.50%(74/80),高于对照组的75.00%(60/80)(P<0.05);2组患者在治疗前到治疗3个月后鼻纤毛传输速率呈上升趋势,且治疗1个月后、治疗后3个月观察组鼻纤毛传输速率高于对照组(P<0.05);与治疗前相比,治疗3个月后2组血清TIgE、ECP水平均降低,且观察组低于对照组(P<0.05);观察组的总不良反应发生率为5.00%(4/80),低于对照组的15.00%(12/80)(P<0.05)。结论鼻渊汤辨证加减治疗慢性鼻-鼻窦炎疗效显著,可增强鼻黏膜清除功能,降低血清TIgE、ECP水平,抑制变态反应,减少并发症发生率。
Objective To explore the effect of Biyuan decoction on the function of nasal mucosa clearance and TIgE,ECP in patients with chronic rhinosinusitis.Methods From February 2018 to December 2019,160 patients with chronic rhinosinusitis admitted to the First Affiliated Hospital of China Medical University were divided into control group(n=80)and observation group(n=80)according to the random number table.The control group was treated with amoxicillin capsule,and the observation group was treated with Biyuan decoction for three months.The clinical effect and complication rate of the two groups were compared after 3 months of treatment.Before treatment,1 month after treatment and 3 months after treatment,the clearance function of nasal mucosa was compared between the two groups.The levels of TIgE and ECP were compared between the two groups before and 3 months after treatment.Results The total clinical effective rate of the observation group was 92.50%(74/80),which was higher than 75.00%(60/80)of the control group(P<0.05).The nasal cilia transmission rate of the two groups of patients showed an upward trend before treatment and 3 months after treatment,and the nasal cilia transmission rate of the observation group was higher than that of the control group after 1 month and 3 months of treatment(P<0.05).Compared with before treatment,the levels of serum TIGE and ECP in 2 groups decreased after 3 months of treatment,and the observation group was lower than the control group(P<0.05).The incidence of adverse reactions in the observation group was 5.00%(4/80),which was lower than that in the control group(15.00%,12/80)(P<0.05).Conclusion Biyuan decoction in the treatment of chronic rhinosinusitis based on syndrome differentiation has a significant effect,which can enhance the function of nasal mucous membrane clearance,reduce serum TIgE and ECP levels,inhibit allergic reactions,and reduce the incidence of complications.
