摘要
目的探讨经皮穴位电刺激与妇科患者术后麻醉镇痛药用量的相关性研究。方法选择2014年8月至2020年10月温州医科大学附属第一医院妇科手术患者62例,美国麻醉医师协会分级Ⅰ~Ⅱ级。术前采用经皮穴位电刺激测定感觉阈和耐受阈。术中使用舒芬太尼诱导并全凭静脉维持,术后使用舒芬太尼自控静脉镇痛。分析术前感觉阈和耐受阈与术后舒芬太尼用量(S)及舒芬太尼总量(SS)的相关性。结果47例患者完成本研究并纳入统计。感觉阈与术后4 h、8 h舒芬太尼用量和术后舒芬太尼总量不相关(P>0.05)。感觉阈与术后12 h、24 h舒芬太尼用量和舒芬太尼总量呈正相关(P<0.05)。耐受阈与术后4 h、8 h、12 h、24 h舒芬太尼用量及舒芬太尼总量均呈负相关(P<0.05),且越接近24 h相关系数越大。结论术前经皮穴位电刺激感觉/耐受阈可以预测妇科手术患者术后疼痛,而耐受阈可能更灵敏。
Objective To explore the correlation study between transcutaneous electrical acupoint stimulation and dosage of narcotic analgesics after gynecological operation.Methods Sixty-two patients who underwent gynecological operation in the First Affiliated Hospital of Wenzhou Medical University from August 2014 to October 2020 were selected,with American Society of Anesthesiologists class I-II.Transcutaneous electrical acupoint stimulation before operation was used to measure sensation threshold and tolerance threshold.Induction with sufentanil and total intravenous maintenance were used during operation.Controlled intravenous analgesia with sufentanil was used after operation.The correlation of preoperative sensory threshold and tolerance threshold with postoperative sufentanil dosage(S)and total sufentanil dosage(SS)was analyzed.Results Forty-seven patients completed the experiment and were included in the statistics.It was found that there was no correlation between the sensory threshold and the sufentanil dosage 4 and 8 hours after surgery and the total postoperative sufentanil dosage(P>0.05).The sensory threshold was positively correlated with the sufentanil dosage 12 h and 24 h after surgery and the total postoperative sufentanil dosage(P<0.05).The tolerance threshold was negatively correlated with the sufentanil dosage 4 h,8 h,12 h and 24 h after surgery and the total postoperative sufentanil dosage(P<0.05),and the closer to 24 h,the greater the correlation coefficient.Conclusion The study indicates that the sensation/tolerance threshold of pre-operative transcutaneous electrical acupoint stimulation can predict postoperative pain in patients undergoing gynecological surgery,and the tolerance threshold may be more sensitive.
