摘要
目的探讨比阿培南联合乙酰半胱氨酸雾化吸入治疗糖尿病合并肺脓肿的临床效果。方法选取2017年1月至2019年12月于在本院住院治疗的120例糖尿病合并肺脓肿患者,随机分为两组,各60例。对照组给予比阿培南抗感染治疗,研究组在对照组基础上加用乙酰半胱氨酸溶液雾化吸入治疗。比较两组炎症指标(CRP、PCT、IL-6)水平、脓液变化情况、临床疗效。结果治疗后,研究组痰液性状评分(0.76±0.53)分,低于对照组的(1.00±0.63)分,痰液量评分(1.25±0.47)分,低于对照组的(1.46±0.59)分。研究组炎症指标CRP(7.50±0.52)mg/L、PCT(0.21±0.02)ng/mL、IL-6(44.07±7.92)ng/L分别低于对照组的(8.93±0.77)mg/L、(0.36±0.03)ng/mL、(46.86±7.16)ng/L,差异有统计学意义(P<0.05)。研究组治疗总有效率为91.67%,高于对照组的78.33%,差异具有统计学意义(P<0.05)。两组治疗期间未见明显的药物不良反应。结论比阿培南联合乙酰半胱氨酸治疗糖尿病合并肺脓肿效果显著,可改善脓液性质,促进引流,提高抗感染作用,值得临床推广应用。
Objective To investigate the clinical effect of atomized inhalation of biapenem combined with acetylcysteine in the treatment of diabetic pulmonary abscess.Methods A total of 120 patients with diabetes mellitus complicated with pulmonary abscess who were hospitalized in our hospital from January 2017 to December 2019 were randomly divided into two groups,with 60 cases in each group.The control group was given biapenem anti-infection therapy,and the study group was given acetylcysteine solution atomized inhalation therapy on the basis of the control group.The levels of inflammatory indicators(CRP,PCT,IL-6)and the changes of pus,and the clinical efficacy were compared between the two groups.Results After treatment,the sputum character score(0.76±0.53)scores in the study group was lower than that in the control group(1.00±0.63)scores,and the sputum volume score(1.25±0.47)scores was lower than that in the control group(1.46±0.59)scores.The inflammatory indexes CRP(7.50±0.52)mg/L,PCT(0.21±0.02)ng/mL and IL-6(44.07±7.92)ng/L in the study group were lower than those in the control group(8.93±0.77)mg/L,(0.36±0.03)ng/mL and(46.86±7.16)ng/L,respectively,the difference was statistically significant(P<0.05).The total effective rate of treatment in the study group was 91.67%,higher than that in the control group(78.33%),and the difference was statistically significant(P<0.05).No obvious adverse drug reactions were observed in the two groups during treatment.Conclusion Biapenem combined with acetylcysteine has a significant effect on the treatment of diabetic pulmonary abscess,which can improve the properties of pus,promote drainage,and improve anti-infective effects.It is worthy of clinical application.
