摘要
目的回顾周边型牙源性纤维瘤的临床病理特点。方法按WHO(2017)牙源性肿瘤分类标准对21例周边型牙源性纤维瘤的临床病理资料进行回顾性分析。结果在21病例中,男性7例,女性14例;平均年龄27岁;上颌牙龈8例,下颌牙龈13例。肿块为局部软组织突出表现。X线片和手术中见3例牙槽骨有吸收改变。5例术后复发。组织学上,肿瘤由致密结缔组织组成,含数量不等的成纤维细胞,所有病例均伴有牙源性上皮团或上皮岛。肿瘤边界尚清,缺少明显包膜。结论周边型牙源性纤维瘤临床表现与牙龈瘤不能区分,通过病理诊断为周边型牙源性纤维瘤,手术切除不彻底易复发。
Objective To review the clinicopathological characteristics of peripheral odontogenic fibroma.Methods A total of 21 cases with peripheral odontogenic fibroma categorized by the WHO classification of odontogenic tumors,cysts and allied lesions(2017)were retrospectively analyzed for their pathological data. Results All the cases with peripheral odontogenic fibroma(8 cases of maxillary gingivae,and 13 cases of mandibular gingivae) were retrospectively analyzed,involving 7 males and 14 females with an average age of 27 years. Peripheral odontogenic fibromas in recruited patients were mainly characterized as local soft tissue masses. Resorption of alveolar bone was detected in 3 cases by X-ray examination and during surgery. Five cases developed postoperative relapse. Histological examination showed that peripheral odontogenic fibroma consisted of dense connective tissues with varying numbers of fibroblasts. Islands and strands of odontogenic epithelium were observed in all recruited cases. The boundary of peripheral odontogenic fibromas was relatively clear without an obvious capsule. Conclusion Peripheral odontogenic fibroma is confused with the epulis,which requires to be pathologically diagnosed. Peripheral odontogenic fibroma easily relapses following an insufficient surgical resection.
