摘要
目的探讨老年人群心房颤动与衰弱表型的相关性。方法选取2019年1—12月于北部战区总医院进行诊疗的205例老年患者为研究对象。根据是否发生心房颤动将其分为心房颤动组(n=38)与非心房颤动组(n=167)。比较两组患者衰弱评估结果。采用多因素Logistic回归分析探讨老年人群衰弱状态发生的独立危险因素。结果心房颤动组患者发生衰弱、衰弱前期比例分别为18.4%(7/38)、63.2%(24/38),均高于非心房颤动组的6.0%(10/167)、29.3%(49/167),差异有统计学意义(P<0.05)。心房颤动组患者步速降低、自我感觉疲劳及自由活动度降低比例均高于非心房颤动组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,心房颤动为老年人群衰弱状态发生的独立危险因素(比值比3.94,95%可信区间1.734~8.073,P<0.05)。结论心房颤动易导致老年人群发生衰弱及躯体功能下降,为老年人群发生衰弱的独立危险因素。
Objective To investigate the correlation between atrial fibrillation and asthenia phenotype in the elderly.Methods A total of 205 elderly patients diagnosed and treated in the General Hospital of Northern Theater Command from January to December in 2019 were selected as the research objects.They were divided into AF group(n=38)and non AF group(n=167).The evaluation results of weakness were compared between the two groups.Multivariate logistic regression analysis was used to explore the independent risk factors of frailty in the elderly.Results The incidence of frailty and prophase of frailty in AF group were 18.4%(7/38)and 63.2%(24/38)respectively, which were higher than 6.0%(10/167)and 29.3%(49/167)in non AF group(P<0.05).The rate of decrease in walking speed, self fatigue and degree of freedom in AF group was higher than that in non AF group, and the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that atrial fibrillation was an independent risk factor for frail state in the elderly(odds ratio 3.94,95%CI:1.734~8.073,P<0.05).Conclusion Atrial fibrillation is a risk factor for frailty in the elderly.
