摘要
目的:建立本院临床医师工作站肠外营养(parenteral nutrition,PN)合理使用支持和监测系统,协助临床医生合理用药,提高工作效率,降低出错率。方法:随机抽取2019年1~12月住院病人的PN医嘱记录,进行回顾性整理分析,针对PN配方合理性(能量、氨基酸量、热氮比、糖脂比、丙氨酰谷氨酰胺用量、鱼油脂肪乳用量等)、PN稳定性(电解质浓度、氨基酸浓度、葡萄糖浓度)、输注途径等问题,将其做为监测内容,应用信息技术开发PN处方管理系统。结果:该系统预置PN处方信息,从医嘱录入环节把控PN合理应用。应用多种监测方式实现对能量、氨基酸量、糖脂比、热氮比、药物配伍、禁忌、稳定性等的监测。结论:PN处方管理系统能够高效、准确、便捷地对处方进行监测,提高临床对PN的合理应用。
Objective:To establish a support and monitoring system for the rational use of parenteral nutrition(PN)in clinicians'workstations in our hospital to assist clinicians in the rational use of drugs,improve work efficiency and reduce error rates.Methods:On randomly selected PN medical order records of inpatients in our hospital from January 2019 to December 2019,retrospective analysis was conducted,aiming at the rationality of PN formula(energy,amino acid,heat-nitrogen ratio,glycolipid ratio,alanyl glutamine dosage,fish oil fat emulsion dosage,etc.),PN stability(electrolyte concentration,amino acid concentration,glucose concentration),infusion route,etc..By setting them as monitoring items,information technology was applied to develop a PN prescription management system.Results:The system preseted PN prescription information to support the rational application of PN from the medical order entry step.A variety of methods were used to monitor the medical orders on energy,amino acid content,glycolipid ratio,heat-nitrogen ratio,drug compatibility,contraindications and stability.Conclusion:The PN prescription management system can monitor medical orders efficiently,accurately and conveniently,and improve the rational application of clinical PN.
