摘要
目的回顾性对比分析胸腔负压引流与胸腔闭式引流治疗胸部外伤所致血气胸的临床疗效。方法选取2017年1月至2019年12月新乡医学院第三附属医院收治的112例分别行胸腔负压引流与胸腔闭式引流治疗的胸部外伤所致血气胸患者作为研究对象,按照胸腔引流术式将其分为胸腔负压引流组与胸腔闭式引流组,每组56例,对比两组患者出血时间(手术开始至胸腔内出血结束时间)、肺复张时间(手术开始至肺完全复张时间)及胸腔内液体累计引流量。结果胸腔负压引流组患者出血时间为(11.23±2.36)d、肺复张时间为(9.84±1.74)d,明显短于胸腔闭式引流组的(14.05±2.59)d和(13.54±2.17)d(t=6.023、9.955,P均<0.001),胸腔内液体累计引流量为(872.9±57.6)mL,明显少于胸腔闭式引流组的(1584.8±89.7)mL(t=49.970,P<0.001)。结论与胸腔闭式引流相比,胸腔负压引流对胸部外伤所致的血气胸治疗效果更佳,能够有效缩短出血时间与肺复张时间。
Objective To retrospectively analyze the curative effect of vacuum thoracic drainage and closed thoracic drainage in the treatment of hemopneumothorax caused by thoracic trauma.Methods 112 patients with hemopneumothorax resulting from thoracic trauma,admitted to the Third Affiliated Hospital of Xinxiang Medical University from January2017 to December 2019 to receive vacuum thoracic drainage and closed thoracic drainage,were selected as research subjects and divided,based on which kind of thoracic drainages they received,into vacuum group and closed group,with 56 cases in each group.The bleeding time(the time from the beginning of the operation to the end of intrathoracic bleeding)and the recruitment maneuver time(from the beginning of the operation to the complete recruitment of the lungs)and cumulative drainage volume of fluid in the chest cavity were compared between the two groups.Results The bleeding time and the lung recruitment maneuver time of patients in the vacuum group were(11.23±2.36)days and(9.84±1.74)days respectively,significantly shorter than the corresponding(14.05±2.59)days and(13.54±2.17)days in the closed group(t=6.023,9.955,bothP<0.001).The cumulative drainage volume of fluid in the chest cavity of patients in the vacuum group was(872.9±57.6)mL,much less than(1584.8±89.7)mL in the closed group(t=49.970,P<0.001).Conclusion Compared with the closed thoracic drainage,the vacuum thoracic drainage has better therapeutic effects on hemopneumothorax caused by thoracic trauma,such as effectively shortening the bleeding time and lung recruitment maneuver time.
