冠心病患者长期右室间隔起搏对心脏结构功能的影响

摘要 目的探讨冠心病患者长期右室间隔起搏对心脏结构功能的影响。方法回顾性分析洛阳市第三人民医院2019年1月至2019年12月收治的62例采用起搏器植入治疗将心室电极置入右室间隔者的临床资料,根据造影检查结果分为冠心病组(30例)与非冠心病组(32例),比较两组起搏器植入前及植入1年后的心室功能参数,二尖瓣反流、三尖瓣反流、主动脉瓣反流、肺动脉高压,并分析长期右室间隔起搏与心脏功能结构的相关性。结果起搏器植入1年后,冠心病组患者的左心房内径(LA)、左心室舒张末内径(LVIDd)、室间隔厚度(IVSd)、左心室后壁厚度(LVPWd)高于起搏器植入前;射血分数(EF)则低于起搏器植入前,差异显著(P<0.05);非冠心病组患者的LA、LVPWd均高于起搏器植入前,差异显著(P<0.05)。冠心病组患者的LA、IVSd、LVPWd高于非冠心病组,而EF低于非冠心病组,差异显著(P<0.05)。起搏器植入1年后,冠心病组患者二尖瓣反流、三尖瓣反流、肺动脉高压发生率均高于起搏器植入前,差异显著(P<0.05);非冠心病组患者的三尖瓣反流率高于起搏器植入前,差异显著(P<0.05)。冠心病组起搏器植入心室起搏比例与LVIDd、IVSd、EF、二尖瓣反流有一定关系(P<0.05)。结论起搏器的植入静脉和时间均会对冠心病患者的心脏功能产生影响。 Objective To investigate the effects of long-term right ventricular septal pacing on cardiac structure and function in patients with coronary heart disease.Methods The clinical data of 62 patients with right ventricular septum implanted by pacemaker from January 2019 to December 2019 in the third people’s Hospital of Luoyang were analyzed retrospectively,the patients were divided into coronary heart disease group(30 cases)and non-coronary heart disease group(32 cases)according to the results of angiography,the parameters of ventricular function,mitral regurgitation,tricuspid regurgitation,aortic regurgitation and pulmonary hypertension were compared before and 1 year after implantation,the relationship between long-term right ventricular septal pacing and cardiac function was analyzed.Results 1 year after pacemaker implantation,the left atrial diameter(LA),left ventricular end-diastolic diameter(LVID),interventricular septum thickness(IVS)and left ventricular posterior wall thickness(LVPW)in patients with CHD were higher than those before pacemaker implantation;Ejection fraction(EF)was significantly lower than that before pacemaker implantation(P<0.05),LA and LVPW were significantly higher in non-CHD group than that before pacemaker implantation(P<0.05).LA,IVSD and LVPW in CHD group were higher than those in non-CHD group,but EF was lower than that in non-CHD group(P<0.05).One year after pacemaker implantation,the incidence of mitral regurgitation,tricuspid regurgitation and pulmonary hypertension in CHD group were significantly higher than that before pacemaker implantation(P<0.05).The tricuspid regurgitation rate in the nonCHD group was significantly higher than that before pacemaker implantation(P<0.05).The rate of pacemaker implantation in CHD group was related to LVID,IVSD,EF and mitral regurgitation(P<0.05).Conclusion Both the implanted vein and the time of pacemaker implantation can affect the cardiac function of patients with coronary heart disease.
