摘要
目的:分析双相情感障碍(BD)躁狂发作患者的甲状腺功能变化情况,为临床干预提供参考依据。方法:采用前瞻性随机试验方法选择2018年8月-2020年8月本院收治的80例BD躁狂发作患者作为研究对象。利用杨氏躁狂评定量表(YMRS)评估其躁狂情况并分组;检测患者甲状腺功能[促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT_(3))、游离甲状腺激素(FT_(4))、三碘甲状腺原氨酸(T_(3))、甲状腺素(T_(4))],分析病情与甲状腺功能的关系。结果:轻度组28例,中度组32例,重度组20例。重度组FT_(3)水平高于轻度组、中度组,且中度组FT_(3)水平高于轻度组,差异有统计学意义(P<0.05);三组TSH、FT_(4)、T_(3)、T_(4)水平比较,差异无统计学意义(P>0.05);BD躁狂发作患者FT_(3)水平表达与YMRS评分呈正相关(r=0.799,P<0.05)。结论:BD躁狂发作患者病情程度与FT_(3)水平呈正相关,检测FT_(3)水平对评估病情具有一定的辅助性作用。
Objective:To analyze the changes of thyroid function in patients with bipolar disorder(BD)manic episodes,and to provide reference for clinical intervention.Method:A prospective randomized trial method was used and selected 80 patients with BD manic episode admitted to our hospital from August 2018 to August 2020 as the research objects.The Young mania rating scale(YMRS)was used to assess their mania status and grouped;the thyroid function[thyroid stimulating hormone(TSH),free triiodothyronine(FT_(3)),free thyroid hormone(FT_(4)),three iodothyronine(T_(3)),thyroxine(T_(4))]was detected,the relationship between the condition and thyroid function was analyzed.Result:There were 28 cases in the mild group,32 cases in the moderate group,and 20 cases in the severe group.The FT_(3) level of the severe group was higher than that of the mild group and the moderate group,and the FT_(3) level of the moderate group was higher than that of the mild group,the differences were statistically significant(P<0.05);there were no significant differences in TSH,FT_(4),T_(3) and T_(4) levels among the three groups(P>0.05);The expression of FT_(3) was positively correlated with YMRS score in patients with BD manic episode(r=0.799,P<0.05).Conclusion:There is a positive correlation between the severity of BD manic episode and the level of FT_(3),and the detection of FT_(3) level has a certain auxiliary effect in evaluating the condition.
