摘要
目的:探究先天性肾上腺皮质增生症(congenital adrenal hyperplasia,CAH)对妊娠的影响。方法:选取2016年1月-2018年1月先天性肾上腺皮质增生症患者60例,经临床确诊对症治疗后成功妊娠30例作为A组,未成功妊娠30例为B组,选取同期收入健康妊娠孕妇30例作为C组,三组患者均比较血清类固醇激素,分析先天性肾上腺皮质增生症与妊娠的相关性。结果:B组17-OHP水平、睾酮高于A组、C组,孕酮、雌酮低于A组和C组;三组比较差异有统计学意义(P<0.05)。17-OHP、睾酮水平与妊娠呈负相关,孕酮、雌酮与妊娠呈正相关,差异有统计学意义(P<0.05)。结论:先天性肾上腺皮质增生症会影响女性正常妊娠,临床可依据类固醇激素改变确诊及开展相应对症治疗,进而改善预后,提高妊娠率。
Objective:To explore the effect of congenital adrenal hyperplasia(CAH)on pregnancy.Method:A total of 60 patients with congenital adrenal hyperplasia from January 2016 to January 2018 were selected.After clinical diagnosis and symptomatic treatment,30 cases of successful pregnancy were selected as group A,30 cases of unsuccessful pregnancy were selected as group B,and 30 cases of healthy pregnant women in the same period were selected as group C.Serum steroid hormones were compared among the three groups,and the correlation between congenital adrenal hyperplasia and pregnancy was analyzed.Result:The 17-OHP level and testosterone in group B were higher than those in group A and group C,while progesterone and estrone in group B were lower than those in group A and group C,there were significant differences among the three groups(P<0.05).The 17-OHP and testosterone levels were negatively correlated with reproductive fertility and pregnancy,while progesterone and estrone were positively correlated with reproductive fertility and pregnancy(P<0.05).Conclusion:Congenital adrenal hyperplasia can affect the normal pregnancy of women,clinical diagnosis can be based on steroid hormone changes and carry out corresponding symptomatic treatment,so as to improve the prognosis and pregnancy rate.
