摘要
目的分析结核感染T细胞酶联免疫斑点法(T-SPOT.TB)、卡介菌纯蛋白衍生物(BCG-PPD)、结核菌素纯蛋白衍生物(TB-PPD)对儿童结核的诊断和评估作用。方法选取2019年4月至2020年4月新乡市结核病防治所检查的90例疑似结核儿童为研究对象,比较T-SPOT.TB、BCG-PPD及TB-PPD 3种检查方法对儿童结核的诊断效能。结果T-SPOT.TB对儿童结核病的诊断灵敏度为85.71%,高于BCG-PPD、TB-PPD(66.67%、61.90%),差异有统计学意义(P<0.05);T-SPOT.TB对儿童结核病的诊断特异度为95.83%,高于BCG-PPD、TB-PPD(77.08%、79.17%),差异有统计学意义(P<0.05)。T-SPOT.TB对≥5岁组的诊断灵敏度为93.10%,高于<5岁组(69.23%),差异有统计学意义(P<0.05);在≥5岁组的儿童中,T-SPOT.TB在结核方面的诊断灵敏度、特异度为93.10%、99.30%,均高于BCG-PPD、TB-PPD(68.97%、68.97%、78.57%、78.57%),差异有统计学意义(P<0.05)。结论T-SPOT.TB对儿童结核的诊断效能较BCG-PPD、TB-PPD高,可作为临床中对于疑似结核患儿的早期筛查技术。
Objective To analyze the role of T-ELISPOT.tuberculosis(T-SPOT.TB),purified protein derivative of BCG(BCG-PPD)and purified protein derivative of tuberculin(TB-PPD)in the diagnosis and evaluation of tuberculosis in children.Methods Ninety children with suspected tuberculosis who came to Xinxiang Tuberculosis Prevention and Control Institute for examination from April 2019 to April 2020 were retrospectively included as the research subjects to compare the diagnostic efficacy of T-SPOT.TB,BCG-PPD and TB-PPD in children with tuberculosis.Results The diagnostic sensitivity of T-SPOT.TB in children was 85.71%,which was higher than that of BCG-PPD and TB-PPD(66.67%and 61.90%)(P<0.05).The diagnostic specificity of T-SPOT.TB for childhood tuberculosis was 95.83%,which was higher than that of BCG-PPD and TB-PPD(77.08%and 79.17%)(P<0.05).The diagnostic sensitivity of T-T-SPOT.TB in≥5 years old group was 93.10%,which was higher than that in<5 years old group 69.23%(P<0.05).In children aged≥5 years,the sensitivity and specificity of T-SPOT.TB in tuberculosis were 93.10%and 99.30%,both higher than those of BCG-PPD and TB-PPD(68.97%,68.97%,78.57%,78.57%)(P<0.05).Conclusion T-SPOT.TB has a higher diagnostic efficiency in children with tuberculosis than BCG-PPD and TB-PPD,and can be used as an early screening technology for children with suspected tuberculosis in clinical practice.
