循经砭术改善血液透析患者运动功能的临床研究 被引量:2

摘要 目的:观察循经砭术对维持性血液透析(maintenance hemodialysis,MHD)患者运动功能的改善作用,为改善MHD患者运动功能提供新的康复方法。方法:采用前瞻随机对照研究方法,依据纳入标准和排除标准选择在我院血液净化中心规律血液透析的患者76例,应用第三方设计的随机软件,随机分对照组和治疗组,每组各38例,对照组接受西医常规治疗,包括规律血液透析及血液透析相关药物治疗,常规碳酸氢盐血液透析每周3次,应用促红素、铁剂、磷结合剂及活性维生素D等纠正血液透析相关并发症。治疗组在西医常规治疗基础上于透析时进行循经砭术治疗,选择双下肢脾胃经,应用捶式砭振器顺经施砭术,并突出足三里、三阴交重点穴位温、点、振、压,以产生“酸、麻、胀”的得气感为最佳。治疗4周为一疗程,观察3个疗程。在治疗前后对入组患者进行运动功能指标心肺耐力、握力、双下肢蹬踏力、双下肢等速肌力力矩峰值测定。结果:(1)心肺耐力采用6 min步行试验检测,两组患者治疗前后进行配对比较,治疗组6 min步行距离优于对照组(P<0.05)。(2)治疗后组间比较,治疗组患者握力、双下肢蹬踏力均较对照组明显提高(P<0.05)。(3)两组治疗前后进行配对,采用单因素方差分析方法,治疗组握力、双下肢蹬踏力、力矩峰值指标高于对照组(P<0.05)。结论:循经砭术可改善血液透析患者心肺耐力及肌力,改善运动功能,安全有效,操作简便,可作为改善MHD患者运动功能的一种中医外治方法。 Objective:To observe the effect of Bian-stone therapy along the meridians on the improvement of motor function in patients with maintenance hemodialysis.Methods:76 hemodialysis patients were selected according to the inclusion and exclusion criteria.They were randomly divided into a control group and a treatment group,each with 38 cases.The control group received conventional Western medicine treatment,and the treatment group received Western medicine.On the basis of conventional treatment,Bian-stone therapy along the meridians was performed during dialysis,4 weeks as a course,and 3 courses were observed.The patients before and after the treatment were indexed for cardiopulmonary endurance,grip strength,pedaling force of both lower limbs,and both lower limbs were isokinetic Determination of peak muscle torque.Results:(1)Cardiopulmonary endurance was measured using a 6-minute walking test.The two groups were compared before and after treatment.The 6-minute walking distance was better in the treatment group than in the control group(P<0.05).(2)Comparison between the groups after treatment,the grip strength and pedaling force of the patients in the treatment group were significantly improved compared with the control group(P<0.05).(3)The two groups were matched before and after treatment,and the univariate analysis of variance method was used.The gripping force,pedaling force,and peak torque of the treatment group were higher than those of the control group(P<0.05).Conclusion:Bian-stone therapy along the meridians can improve the cardiopulmonary endurance,muscle strength and exercise function of hemodialysis patients.It can be used as a external treatment method to improve the motor function of MHD patients.
