摘要
目的基于美国国立癌症研究所“监测、流行病学和结果”(the Surveillance,Epidemiology and End Results,SEER)数据库探讨不同年龄层次恶性周围神经鞘瘤(malignant peripheral nerve sheath tumor,MPNST)患者的预后。方法从SEER数据库中选择1973—2014年被诊断为MPNST的患者。使用Kaplan-Meier法和COX比例风险模型分析年龄与预后的关系。并在不同亚组中分析不同年龄段死亡风险的差异。结果COX分析结果显示,与≥60岁年龄组相比,<20岁组HR=0.47(95%CI:0.33~0.66)、20~39岁组HR=0.61(95%CI:0.49~0.77)、40~50岁组HR=0.55(95%CI:0.44~0.70)。在肿瘤分期为“局部”和“区域”的患者中,年龄是患者预后的独立预测因素(P<0.01),分期为“远处转移”的患者不同年龄组间死亡风险的差异无统计学意义(P>0.05)。在不同原发部位中,年龄均是患者预后的独立预测因素(P<0.01)。结论年龄是MPNST患者独立的预后预测因素。≥60岁年龄组的患者死亡风险显著高于其他年龄组。
Objective To investigate the prognosis of malignant peripheral nerve sheath tumor(MPNST)in different age groups based on the Surveillance,Epidemiology,and End Results(SEER)database.Methods A total of 689 patients diagnosed with MPNST from 1973 to 2014 were collected from SEER database.Kaplan-Meier method and COX proportional hazards model were used to analyze the relationship between age and prognosis;and the mortality risk at different age groups was also analyzed.Results COX analysis showed that compared with the age group≥60 years,the HR=0.47(95%CI:0.330.66)in the<20 years old,the HR=0.61(95%CI:0.490.77)in the 2039 years group,and the HR=0.55(95%CI:0.440.70)in the 4050 years group.In patients with local and regional tumor,age was an independent predictor of prognosis(P<0.01);while there was no significant difference in death risk among different age groups of patients with distant metastasis(P>0.05).Age was an independent predictor of prognosis in different primary sites(P<0.01).Conclusion Age is an independent predictor of prognosis in patients with MPNST,and the mortality risk of patients aged≥60 years is higher than that in other age groups.
