摘要
目的探讨应用腹股沟皮瓣游离移植治疗儿童、青少年中小面积颈部瘢痕的可行性。方法回顾分析2019年7月至2020年12月间接受腹股沟皮瓣游离移植治疗颈部瘢痕的儿童、青少年患者9例。患者瘢痕累及范围局限于颈前区或一侧颈外侧区,存在不同程度的颈部活动障碍。术中先切除、松解颈部瘢痕,再按创面大小和形态设计并切取腹股沟皮瓣,显微吻合面动静脉与旋髂浅动静脉,供区创面直接缝合。结果本组有1例出现皮瓣移植后远端因静脉回流欠佳而小面积浅表组织坏死,换药后创面愈合;2例出现皮瓣供区创面愈合延迟。本组患者术后随访6~12个月,颈部外观及功能均得到明显改善,1例进行了皮瓣蒂部修薄术。结论腹股沟皮瓣游离移植修复儿童、青少年颈部中小面积瘢痕增生挛缩畸形,可明显改善患者功能及外观。
Objective To explore the feasibility of applying free groin flap transplantation to treat small and medium-sized cervical scar in children and adolescent patients.Methods Nine cases of children and adolescents with cervical scars from July 2019 to December 2020 were retrospectively reviewed and were treated by free groin flap transplantation.The scars involved were limited to the anterior neck or the lateral neck,and there were varying degrees of cervical dysfunction.During the operation,the cervical scar was excised and loosened,and then the groin flap was designed and harvested according to the size and shape of the cervical wound.The facial artery and vein were anastomized with the superficial iliac circumflex artery and vein,and the donor site was sutured directly.Results In the group of 9 children and adolescent patients,one transplanted flap had a small area of superficial tissue necrosis at the distal ending due to poor venous circulation which was successfully treated by dressing change.Two patients had delayed wound healing at the donor site.The postoperative follow-up was from 6 months to 1 year.The appearance and function of the neck were both significantly improved in all 9 patients.One patient underwent flap debulking at the pedicle area.Conclusion The groin flap can be freely transplanted to improve the cervical appearance and function of children and adolescents who have small and medium-sized cervical scars.
