摘要
目的评价中国成人血清25(OH)D水平与骨密度的相关性。方法检索万方、知网、维普等数据库,检索时间从建立数据库至2020年1月30日。依照美国医疗保健研究与质量局(Agency for Healthcare Research and Quality,AHRQ)的观察性研究质量评价标准进行质量评价,结局指标使用R 3.6.6进行合并。结果最终入选24篇文献,总样本量为6305例,文献质量等级总体较高。Meta分析结果显示,血清25(OH)D水平与腰椎L1~4、全髋、股骨颈骨密度的Z值有显著性,分别为[Fisher’Z=0.16,95%CI(0.15,0.19),P<0.05]、[Fisher’Z=0.12,95%CI(0.09,0.15),P<0.05]、[Fisher’Z=0.15,95%CI(0.11,0.12),P<0.05],相关系数r分别为0.16、0.12、0.15。纳入的研究无明显发表性偏倚,敏感性分析认为结果具有稳定性。结论中国成人血清25(OH)D水平与骨密度存在显著的正相关关系,相关程度为极弱相关,鉴于总体文献循证级别不高,未来仍需要进行进一步验证。
Objective To evaluate the correlation between serum 25(OH)D level and bone mineral density in Chinese adults.Methods Retrieval the databases of Wanfang,CNKI,VIP,time from the establishment until January 30,2020.The quality of the literatures were assessed by the Agency for Healthcare Research and Quality standard.Meta-analysis was conducted via the software R 3.6.3.Results A total of 24 literatures were included,the total sample size was 6305,and the literature quality evaluation grade was relatively high.Meta analysis result:Serum 25(OH)D level was significantly correlated with bone mineral density Fisher’s Z value of lumbar spine 1-4 vertebrae,total hip and femoral neck,which were[Fisher’Z=0.16,95%CI(0.15,0.19),P<0.05],[Fisher’Z=0.12,95%CI(0.09,0.15),P<0.05],[Fisher’Z=0.15,95%CI(0.11,0.12),P<0.05],summary r was 0.16,0.12 and 0.15,respectively.There was no distinct publication bias,and the sensitive analysis result were stable.Conclusion There is a significant positive correlation between serum 25(OH)D level and bone mineral density in adults in China,and the degree is extremely weak.Taking the low level of evidence into consideration,further strict academic proof is needed.
