摘要
目的分析三腔二囊尿管经肛门直肠腔冲洗预防结直肠癌患者术后结直肠吻合口瘘的效果及对患者术后生活质量的影响。方法选择我院2017年5月至2020年5月收治的128例行手术治疗的结直肠癌患者,根据治疗方案的差异分为对照组与研究组,每组64例。对照组术后采用常规治疗,研究组术后采用三腔二囊尿管经肛门直肠腔冲洗治疗。观察比较两组患者术后1个月内结直肠吻合口瘘的发生情况、腹痛缓解时间、血糖水平、抑郁及焦虑状态、术后不同阶段生活质量。结果研究组术后1个月内结直肠吻合口瘘发生率、腹痛缓解时间、血糖水平、汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分均优于对照组,组间差异有统计学意义(P<0.05)。术后1、2、3、4周,研究组生活质量评分均高于对照组,组间差异有统计学意义(P<0.05)。结论临床针对结直肠癌术后患者,可采用三腔二囊尿管经肛门直肠腔冲洗方案治疗,不仅能够降低患者结直肠吻合口瘘发生率,且治疗方案无痛无创,可减轻患者的疼痛感,降低患者的血糖水平,进而优化患者的情绪状态,改善其生活质量。
Objective To analyze the effect of three-cavity two-cystic urinary catheter through anorectal cavity flushing to prevent postoperative colorectal anastomotic fistula in patients with colorectal cancer and its impact on the quality of life of patients after surgery.Methods The 128 patients with colorectal cancer who were treated in our hospital from May 2017 to May 2020 who underwent surgical treatment were selected and divided into a control group and a study group,with 64 cases each according to the difference in treatment plan.The control group received conventional treatment after operation,and the study group received three-cavity two-cystic urinary catheter flushing treatment through anorectal cavity after operation.Observation and comparison of the incidence of colorectal anastomotic fistula within 1 month after surgery,abdominal pain relief time,blood sugar level,depression and anxiety status,and quality of life at different stages of the operation in the two groups.Results The incidence of colorectal anastomotic fistula,abdominal pain relief time,blood glucose level,Hamilton Depression Scale(HAMD)score,Hamilton Anxiety Scale(HAMA)score within 1 month after operation in the study group were better than those of the control group,and there were statistically significance between the groups(P<0.05).At 1,2,3,and 4 weeks after surgery,the quality of life scores in the study group were higher than those in the control group,and the difference between the groups was statistically significant(P<0.05).Conclusion Clinically for patients after colorectal cancer surgery,three-cavity two-cystic urinary catheter can be used for treatment through anorectal cavity irrigation,which can not only reduce the incidence of colorectal anastomotic fistula,but also the treatment plan is painless and non-invasive,which can reduce the pain of patients.Reduce the patient’s blood sugar level,thereby optimizing the patient’s emotional state and improving their quality of life.
