摘要
目的观察冠心病患者接受冠状动脉支架介入治疗的预后效果。方法选取2016年2月至2018年5月于我院接受冠状动脉支架介入术治疗的270例冠心病患者为研究对象。对所有患者展开为期3年的随访,回顾性分析其临床资料、不良心脏事件发生情况以及预后效果的相关影响因素。结果270例患者均顺利完成随访,其中87例(32.22%)发生不良心脏事件(包括死亡8例,非致命性心肌梗死18例,心绞痛复发52例,二次介入治疗9例)。患者术后1年的生存率为98.15%,术后3年的生存率为97.04%。引起不良心脏事件的单因素包括患者的年龄、家族史、高血压史、糖尿病史、吸烟史、病变血管支数以及支架数目;多因素分析结果显示,年龄、高血压史、糖尿病史、吸烟史、病变血管支数以及支架数目为引发不良心脏事件的独立因子(P<0.05)。结论对于接受冠状动脉支架介入治疗的冠心病患者,其预后效果会受年龄、病史、病变血管支数等因素的影响,临床应采取针对性的干预措施以促进患者预后效果及生活质量的提高。
Objective To observe the prognostic factors of coronary heart disease patients after coronary stent intervention.Methods From February 2016 to May 2018,270 cases of coronary heart disease patients who received coronary stent intervention in our hospital were selected as study objects.All patients were followed up for three years,and clinical data,occurrence of adverse cardiac events and prognostic factors were retrospectively analyzed.Results All the 270 patients were followed up successfully,and 87 patients(32.22%)had adverse cardiac events(including 8 deaths,18 non-fatal myocardial infarction,52 cases of angina pectoris recurrence,and 9 cases of secondary intervention).The one-year survival rate was 98.15%,and the three-year survival rate was 97.04%.The single factors causing adverse cardiac events mainly include the patient’s age,family history,history of hypertension,history of diabetes,history of smoking,number of diseased blood vessels and number of stents,etc.Multi-factor analysis showed that age,history of hypertension,history of diabetes,history of smoking,number of diseased blood vessels and number of stents were all independent factors leading to adverse cardiac events(P<0.05).Conclusion For patients with coronary heart disease who receive coronary stent intervention,the prognostic effect will be affected by age,medical history,number of diseased vessels and other factors.Targeted intervention measures should be taken in clinical practice to improve the prognostic effect and quality of life of patients.
