摘要
在内分泌科建立糖尿病“7日”教学模式,对住院糖尿病患者进行健康档案建立、入院评估,住院期间按其指定时间表进行实施并反馈,出院后进行随访,为患者动态制定个体化糖尿病管理方案及规范化的生活方式。以提高患者自我管理依从性及自身糖尿病控制水平,并改善患者健康行为,进一步加强血糖控制。
The Department of Endocrinology has established a diabetes"7-day"teaching model to establish health records and admission assessments for in-patients with diabetes.During the hospitalization period,they will be implemented according to their designated timetable and feedback.Follow-up will be carried out after discharge,to dynamically develop individualization diabetes management plan and standardized lifestyle for patients.In order to improve patients'self-management compliance and their own diabetes control level,and improve patients'health behaviors,further strengthen blood glucose control.
