摘要
目的探讨25-(OH)D、FIB、PCT及sTREM-1水平与儿童感染性肺炎的相关性。方法以自2018年8月-2020年3月于我院收治入院的180例感染性肺炎儿童为研究对象,根据其病情严重程度将其分为轻症组(n=49)和严重组(n=131),同时收集同期体检健康儿童为对照组(n=135),对比三组研究对象血清25-(OH)D、血浆纤维蛋白原(Fib)、降钙素原(PCT),以及血清可溶性髓样细胞触发受体-1(sTREM-1)水平,并进行相关性分析。结果严重组PCT、FIB和sTREM-1均高于轻症组及对照组,25-(OH)D低于轻症组及对照组,差异显著(P<0.05);轻症组PCT、FIB和sTREM-1均高于对照组,25-(OH)D低于对照组,差异显著(P<0.05)。严重组25-(OH)D缺失率明显高于轻症组及对照组,差异显著(P<0.05);轻症组25-(OH)D缺失率明显高于对照组,差异显著(P<0.05)。经Pearson相关性分析,感染性肺炎儿童病情严重程度与25-(OH)D呈显著负相关(r=-0.552,P<0.05),与FIB、PCT及sTREM-1水平呈正相关(r=0.571、0.587、0.521、0.598,P<0.05)。结论25-(OH)D、FIB、PCT以及sTREM-1与儿童感染性肺炎的发生具有一定的相关性,同时可反映肺炎的严重程度。故监测25-(OH)D、FIB、PCT及sTREM-1水平在评估感染性肺炎病情严重程度方面具有重要的临床价值。
Objective To investigate the correlation between 25-(OH)D,FIB,PCT levels and childhood infectious pneumonia.Methods 180 children with infectious pneumonia admitted to our hospital from August 2018 to March 2020 were used as the research objects.According to their severity,they were divided into the mild group(n=49)and the severe group(n=131).At the same time,healthy children were collected for physical examination during the same period as the control group(n=135).The serum levels of 25-(OH)D,plasma fibrinogen(fibrinogen,Fib),procalcitonin(PCT)and(soluble triggering receptor expressed on myeloid cells-1(sTREM-1)in the three groups were tested and compared and correlation analysis was performed.Results The levels of PCT,FIB and sTREM-1 of the severe group were higher than those of the mild group and the control group,25-(OH)D was lower in the severe group than in the mild group and the control group(P<0.05).The levels of PCT,FIB and sTREM-1 in the mild group were higher than those in the control group,and 25-(OH)D was lower in the mild group than the control group(P<0.05).The deletion rate of 25-(OH)D in the severe group was significantly higher than that in the mild group and the control group(P<0.05),and it was significantly higher in the mild group than in the control group(P<0.05).According to Pearson correlation analysis,the severity of patients with infectious pneumonia was significantly negatively correlated with 25-(OH)D(r=-0.552,P<0.05),and was positive correlated with FIB,PCT and sTREM-1 levels(r=0.571,0.587,0.521,0.598,P<0.05).Conclusion 25-(OH)D,FIB,PCT and STREM-1 have a certain correlation with the occurrence of childhood infectious pneumonia,and they can reflect the severity of pneumonia.Therefore,monitoring the levels of 25-(OH)D,FIB,PCT and sTREM-1 has important clinical value in assessing the severity of infectious pneumonia.
