摘要
目的探究老年糖尿病患者低血糖恐惧与自我管理、应对方式的相关性及护理对策。方法选择医院2019年8月—2020年1月收治的老年糖尿病患者62例作为研究对象,采用低血糖恐惧感量表、自我管理行为量表及医学应对问卷进行调查,通过Pearson相关性分析明确低血糖恐惧与自我管理、应对方式的相关性,多元线性回归分析筛选老年糖尿病患者低血糖恐惧的相关影响因素,据此确定护理对策。结果结果显示,老年糖尿病患者低血糖恐惧评分为28.43±3.64分,无或轻度恐惧35例,中重度恐惧27例;Pearson相关性检验显示,老年糖尿病患者低血糖恐惧与自我管理呈负相关性(r=-0.215,P=0.028),与回避应对、屈服应对呈正相关性(r=0.362,P<0.001;r=0.336,P=0.002);多元线性回归分析显示,老年糖尿病患者低血糖恐惧的相关影响因素有病程、糖尿病健康教育、饮食行为依从性、低血糖频次、低血糖严重程度。结论部分老年糖尿病患者伴有低血糖恐惧,其与自我管理呈负相关性,与回避应对、屈服应对呈正相关性,且受多因素影响,需强化临床护理干预。
Objective To explore the correlation between the fear of hypoglycemia and self-management,coping styles in elderly diabetic patients,formulate nursing countermeasures accordingly.Methods A total of 62 elderly diabetic patients admitted to the hospital from August 2019 to January 2020 were selected as the study objects.The hypoglycemia fear scale,self-management behavior scale and medical response questionnaire were used to investigate.Pearson correlation analysis was used to clarify the correlation between hypoglycemia fear and self-management,coping style,and multiple linear regression analysis was used to screen the related influencing factors of hypoglycemia fear in elderly diabetic patients,and determined nursing countermeasures accordingly.Results The results showed that the score of hypoglycemia fear in elderly diabetic patients was 28.43±3.64 points,35 cases had no or mild fear,and 27 cases had moderate or severe fear.The Pearson correlation test showed that the fear of hypoglycemia in elderly diabetic patients was negatively correlated with self-management(r=-0.215,P=0.028),and positively correlated with avoidance coping and yield coping(r=0.362,P<0.001;r=0.336,P=0.002).Multiple linear regression analysis showed that the related influencing factors of the fear of hypoglycemia in elderly diabetic patients included the course of disease,diabetes health education,compliance with dietary behavior,frequency of hypoglycemia,and severity of hypoglycemia.Conclusion Some elderly diabetic patients were associated with fear of hypoglycemia,which was negatively correlated with self-management,and positively correlated with avoidance coping and yield coping,and was affected by multiple factors.Therefore,it was necessary to strengthen clinical nursing intervention.
