摘要
目的探究重症监护室机械通气患儿脱机困难现况及其危险因素。方法选择医院重症监护室2017年12月—2019年12月收治的机械通气患儿222例为研究对象,将56例发生脱机困难的患者纳为病例组,脱机成功的166例患者纳为对照组。收集患儿性别、年龄、合并肺部基础疾病、1年内入院次数等病历资料,采用多因素Logistic回归分析调查机械通气患儿脱机困难的危险因素。结果单因素及多因素分析显示,合并肺部基础疾病、血清白蛋白水平、呼吸机相关性肺炎、通气时间、护士年资是机械通气患儿脱机困难的促进因素(P<0.05)。结论部分机械通气患儿存在脱机困难风险,其受多因素影响,护理人员需高度警惕,强化风险因素识别、干预。
Objective To explore the current status and risk factors of weaning di fficulties in children patients with mechanical ventilation in the intensive care unit.Methods A total of 222 mechanically ventilated children patients admitted to the hospital’s intensive care unit from December 2017 to December 2019 were selected as the study objects.The 56 patients with di fficulty in weaning were included in a case group,and the other 166 patients who were successfully weaned were included in a control group.The medical records of the children’s gender,age,combined pulmonary diseases,and the number of hospital admissions within a year was collected,and multivariate Logistic regression analysis was used to investigate the risk factors of weaning di fficulties in mechanically ventilated children.Results Univariate and multivariate analysis showed that the combined with underlying lung disease,serum albumin level,ventilator-associated pneumonia,ventilation time,and nurse’s seniority were the contributing factors to the difficulty of weaning in children patients with mechanically ventilation(P<0.05).Conclusion Some children patients with mechanical ventilation were at risk of di fficulty in weaning,which was af fected by multiple factors.Nursing staf f need to be highly vigilant and strengthen the identification and intervention of risk factors.
