摘要
目的分析血清D-二聚体、纤维蛋白原(FIB)水平与肺癌患者远端转移的关系。方法回顾性分析2017年1月至2020年6月我院收治的120例肺癌患者临床资料,根据CT及同位素骨扫描结果分为远端转移组48例与非远端转移组72例。检测2组血清D-二聚体、FIB水平,分析两指标与肺癌患者远端转移的关系。结果Ⅲ~Ⅳ期患者血清D-二聚体、FIB水平均高于Ⅰ~Ⅱ期者,差异有统计学意义(P<0.05);远端转移组血清D-二聚体、FIB水平均高于非远端转移组,差异有统计学意义(P<0.05);线性回归结果显示,血清D-二聚体、FIB与肺癌患者远端转系息息相关(P<0.05),得到线性回归方程:Y=2.375+0.630X;Y=2.375+0.192X;D-二聚体、FIB及联合检测的曲线下面积(AUC)分别为0.807、0.768、0.826,均>0.7,联合诊断的AUC更大、敏感度更高,诊断价值更高。结论血清D-二聚体、FIB水平与肺癌远端转移密切相关,早期检测血清D-二聚体、FIB水平对肺癌远端转移具有指导价值,且联合检测的价值更高。
Objective To analyze the relationship between serum D-dimer,fibrinogen(FIB)levels and distal metastasis in patients with lung cancer.Methods The clinical data of 120 patients with lung cancer admitted to our hospital from January 2017 to June 2020 were retrospectively analyzed,they were divided into distal metastasis group(n=48)and non-distal metastasis group(n=72)according to CT and isotope bone scan results.The serum D-dimer and FIB levels of the two groups were detected,and the relationship between the two indexes and the distal metastasis of lung cancer patients was analyzed.Results The serum levels of D-dimer and FIB in stageⅢ-Ⅳpatients were higher than those in stageⅠ-Ⅱpatients,and the difference was statistically significant(P<0.05).Serum D-dimer and FIB levels in distal metastasis group were higher than those in non-distal metastasis group,the difference was statistically significant(P<0.05).The linear regression results showed that serum D-dimerand FIB were closely related to the distal metastasis of lung cancer patients(P<0.05),and the linear regression equation was obtained as follows:Y=2.375+0.630X;Y=2.375+0.192X,and the area under curve(AUC)of D-dimer,FIB and combined detection were 0.807,0.768 and 0.826,respectively,all>0.7,the combined diagnosis had a larger AUC,higher sensitivity,and higher diagnostic value.Conclusion Serum D-dimer and FIB levels are closely related to distal metastasis of lung cancer.Early detection of serum D-dimer and FIB has guiding value for distal metastasis of lung cancer,and the combined detection is of higher value.
