零角度上挑扩皮法在PICC置管改良赛丁格技术中的应用 被引量:2

摘要 目的探讨零角度上挑扩皮法在PICC置管改良赛丁格技术中的应用效果。方法将2018年1月至2019年8月我科收治的108例需采用超声联合改良赛丁格技术行PICC置管的肿瘤患者,通过随机数字表法分成两组。在扩皮操作时,对照组54例患者采用纵切扩皮法,试验组54例患者采用零角度上挑扩皮法。比较两组患者一次扩皮送鞘成功率、穿刺部位渗血情况、扩皮疼痛程度、7 d内的导管维护频次。结果试验组一次扩皮送鞘成功率为92.59%,高于对照组的87.04%(P<0.05);试验组扩皮即刻、置管后24 h穿刺部位渗血情况少于对照组,置管后7 d内的维护频次低于对照组(P<0.05)。而两组患者扩皮时疼痛程度比较,差异无显著性(P>0.05)。结论零角度上挑扩皮法送插管鞘成功率高,创伤小,出血少,减少了维护频次,减轻了患者经济负担及护理工作量。 Objective To explore the application effect of the zero-angle upward pick-and-expand skin method in PICC tube placement improved Sedinger technique.Methods From January 2018 to August 2019,108 tumor patients in our department who required PICC intubation with ultrasound combined with modified Sedinger technology were divided into two groups by random number table method.During the skin expansion operation,54 patients in the control group used the longitudinal incision and expansion method,and the 54 patients in the test group used the zero-angle uplift skin expansion method.The success rate of one-time expansion and delivery of the sheath,the bleeding at the puncture site,the degree of pain during expansion,and the frequency of catheter maintenance within 7 days were compared between the two groups.Results The successful rate of one-time expansion and delivery of the sheath in the experimental group was 92.59%,which was higher than 87.04%in the control group(P<0.05);the blood oozing at the puncture site immediately after the expansion and 24 hours after the catheterization in the experimental group was less than that in the control group.The maintenance frequency in the last 7 days was lower than that of the control group(P<0.05).However,there was no significant difference in the pain degree of the two groups of patients during skin expansion(P>0.05).Conclusion The zero-angle upward pick-and-expansion method has a high success rate for delivery of the intubation sheath,less trauma,less bleeding,reduced maintenance frequency,and reduced the economic burden of patients and the workload of nursing care.
