摘要
目的:探讨过渡性护理模式(transitional care model,TCM)在脑卒中瘫痪患者住院过程中的应用效果。方法:采用随机抽样法,选取2019年2月-2020年2月本科收治的100例80岁以下的脑卒中偏瘫患者为研究对象。按照入组先后时间排序,偶数入选试验组,奇数入选对照组,每组50例。对照组按照脑卒中护理常规进行护理,研究组在常规护理基础上实施TCM。比较两组的自护能力、家属对患者的管理效果及护理满意度情况。结果:干预前,两组ESCA评分比较,差异均无统计学意义(P>0.05);干预后,试验组自我概念、自护责任感、自我护理技能、健康知识水平评分均高于对照组,差异均有统计学意义(P<0.05)。试验组家属对患者的管理能力优于对照组,差异有统计学意义(P<0.05)。试验组的满意度虽高于对照组,但差异无统计学意义(P>0.05)。结论:TCM可以对脑卒中偏瘫患者起到较好的疗效,主要表现为提升患者自护的能力,提高家属对患者的管理效果。其将有望成为脑卒中偏瘫患者理想的护理模式,值得进一步临床推广应用。
Objective:To explore the effect of transitional care model(TCM)in the hospitalization of patients with cerebral apoplexy paralysis.Method:A total of 100 cerebral apoplexy paralysis under the age of 80 who admitted to our university from February 2019 to February 2020 were selected as the research objects by random sampling method.According to the sequence of enrollment time,the even number was selected into the experimental group,and the odd number was selected into the control group,50 cases in each group.The control group received nursing care according to the routine of stroke nursing,the study group implemented TCM on the basis of routine nursing.The self-care ability,the management effect of family members on patients and the nursing satisfaction of the two groups were compared.Result:Before intervention,there were no significant differences in ESCA scores between two groups(P>0.05).fter intervention,the scores of self-concept,self-care responsibility,self-care skills and health knowledge in the experimental group were all higher than those in the control group,the differences were statistically significant(P<0.05).The family members of the test group had better ability to manage patients than the control group,the difference was statistically significant(P<0.05).The satisfaction of experimental group was higher than that of control group,but the difference was not statistically significant(P>0.05).Conclusion:TCM can have a good effect on cerebral apoplexy paralysis,mainly manifested as improving the ability of patients to self-care,improving the management effect of family members on patients.It is expected to be an ideal nursing mode for stroke patients with hemiplegia and is worthy of further clinical promotion and application.
