摘要
目的:分析血清淀粉样蛋白A(serum amyloid A,SAA)、中性粒细胞CD4(neutrophil CD4,nCD4)对白血病合并细菌感染的诊断价值。方法:选取2018年7月-2020年7月于本院诊治的47例白血病合并细菌感染患者(感染组)与47例白血病未合并感染患者(对照组)作为研究对象,检测两组血清SAA、C反应蛋白(C-reactive protein,CRP)、降钙素原(procalcitonin,PCT)与nCD4指数;分析各指标与白血病合并细菌感染的相关性;分析SAA、nCD4指数及两者联合对白血病合并细菌感染的诊断价值。结果:感染组SAA、CRP、PCT均高于对照组(P<0.05)。感染组的nCD4指数为(176.32±2.58)高于对照组的(51.58±3.19)(P<0.05)。白血病合并细菌感染与SAA、CRP、nCD4指数、PCT均具有相关性(P<0.05)。ROC曲线结果显示SAA、nCD4指数及两者联合诊断白血病合并细菌感染的AUC分别为0.724、0.745和0.867。结论:SAA、nCD4在白血病合并细菌感染患者中呈现高表达状况,与细菌感染显著相关,两者联合对白血病合并细菌感染具有较高的诊断价值。
Objective:To analysis the diagnostic values of serum amyloid A(SAA)and neutrophil CD4(nCD4)for leukemia combined with bacterial infection.Method:A total of 47 patients with leukemia complicated with bacterial infection(infected group)and 47 leukemia patients without complicated with bacterial infection(control group)diagnosed and treated in our hospital from July 2018 to July 2020 were selected as the research subjects.SAA,C-reactive protein(CRP),procalcitonin(PCT)and nCD4 index in the two groups were detected.The correlation between each index and leukemia complicated bacterial infection was analyzed;the diagnostic value of SAA,NCD4 index and their combination in leukemia complicated bacterial infection was analyzed.Result:The SAA,CRP and PCT of the infection group were higher than those of the control group(P<0.05).The nCD4 index of the infection group was(176.32±2.58),which was higher than(51.58±3.19)of the control group(P<0.05).Leukemia complicated bacterial infection was correlated with SAA,CRP,nCD4 index and PCT(P<0.05).The ROC curve showed that the AUC of SAA,nCD4 index and the combined diagnosis of leukemia complicated with bacterial infection were 0.724,0.745 and 0.867,respectively.Conclusion:SAA and nCD4 are highly expressed in patients with leukemia combined by bacterial infection,and are significantly related to bacterial infection.The combination of SAA and nCD4 has high diagnostic value for leukemia complicated with bacterial infection.
