能谱CT定量参数鉴别良性前列腺增生与膀胱癌 被引量:2

Identification of benign prostatic hyperplasia and bladder cancer based on spectral CT quantitative parameters

摘要 目的探讨能谱CT定量参数鉴别良性前列腺增生(BPH)与膀胱癌的价值。方法回顾性分析甘肃省中医院2014年1月—2017年12月经病理学诊断证实的膀胱癌患者27例、BPH患者22例的相关资料。所有患者术前均行全腹部能谱CT三期增强扫描,分别于动脉期和静脉期在病灶内3个不同层面放置感兴趣区取平均值,分析在40~140 keV单能量下同一期像BPH及膀胱癌的CT值及CT能谱曲线差异,计算BPH及膀胱癌的能谱曲线斜率和标准化碘浓度(NIC)。所有测量及计算数据采用独立样本Mann-Whitney U检验。结果动脉期膀胱癌和BPH,在40~60 keV单能量下CT值差异具有统计学意义(P<0.05),其中,在40 keV单能量下BPH与膀胱癌差异最大;能谱曲线斜率BPH明显低于膀胱癌(P<0.01);NIC差异有统计学意义(P<0.01)。结论能谱CT定量参数较普通混合能量CT,是鉴别BPH与膀胱癌更为有效的方法。 Objective To evaluate the value of spectral CT quantitative parameters in the identification of benign prostatic hyperplasia(BPH) and bladder cancer. Methods Clinical data of 27 patients with bladder cancer and 22 patients with BPH admitted by pathological diagnosis in Gansu Provincial Hospital of TCM from January 2014 to December 2017 were retrospectively analyzed. All patients underwent the whole abdominal spectral CT three-phase enhanced scan before operation, with locations of the region of interest at 3 different levels in the lesion at the arterial and venous phase. The difference of CT value and spectral CT curve of BPH and bladder cancer in the same phase under 40~140 keV single energy were analyzed. The spectral curve slope and the normalized iodine concentration(NIC) of BPH and bladder cancer calculated. All the measured and calculated data were used by independent-sample Mann-Whitney U test. Results The monochromatic CT value of both bladder cancer and BPH at the 40~60 keV level in the arterial phase were statistically significant(P< 0.05), and the difference between BPH and bladder cancer was the largest under 40 keV monochromatic CT value. The spectral curve slope BPH was obviously lower than that of bladder cancer(P< 0.01), with a significant statistical difference in NIC(P< 0.01). Conclusion The spectral CT quantitative parameters can be used to provide a more effective method for the identification of BPH and bladder cancer than mixed energy CT.
